Referral Management · Receive, review, and process referrals from hospitals, physicians, facilities, and community partners. Verify completeness of referral documentation (face-to-face, orders, insurance, demographics, diagnosis codes). Communicate promptly with referral sources to obtain missing information or clarification. Patient Eligibility & Verification · Confirm patient eligibility and coverage with payers (Medicare, Medicaid, Managed Care, Commercial Insurance). Validate primary care physician and attending provider credentials and NPI. Confirm patient location, service area, and insurance plan network participation. Coordination & Communication · Collaborate with clinical intake nurses, scheduling, and field staff for start-of-care assignments. Communicate admission readiness and patient details to clinicians and case managers. Compliance & Documentation · Confirm that physician orders, F2F (Face-to-Face), and consent forms are complete before admission. Tracking & Reporting · Maintain referral logs and update intake tracking dashboards daily. Department Overview Family Medicine at Scappoose is a primary care Family Medicine clinic and Rural Health Center (RHC). Our PICU offers high-level intensive care for more than 450 critically ill children each year. Our providers have the expertise to care for children with: Acute critical illness, serious injuries from accidents, severe neurological illness or injury, organ failure, those who have undergone complex surgery. This expert care includes advanced therapies in invasive and non-invasive respiratory support, sophisticated monitoring techniques, and administering continuous infusions that require close medical supervision. Some of our patients require 1:1 nursing care, while other patients are part of a 1:2 nurse:patient ratio.Our rounds and approach to care are always multidisciplinary and we focus on evidence based practices to provide the most up to date, highest quality care. All shifts are 12 hrs and utilize a self- scheduling model. In accordance with national standards of nursing practice and within the guidelines and policies and procedures of both Maine Medical Center and the Maine State Board of Nursing, the Clinical Nurse is responsible for assessment, diagnosis, outcome identification, planning, implementation, documentation, and evaluation of the effects of nursing care. Care delivery and coordination of patient and family care begins with the initial contact with the health care system and may continue across the continuum. Services may be provided on an inpatient, outpatient, ambulatory, and/or community basis. Required Minimum Knowledge, Skills, And Abilities (KSAs) Education: Bachelor of Science Degree in Nursing (BSN) preferred. Evidence of 10 hours of ongoing education per year License/Certifications: Current license to practice as a Registered Professional Nurse in the State of Maine. Certification in relevant specialty preferred. Experience: At least 1 year prior RN work experience. At least 1 year prior RN work experience. Prior PICU experience strongly preferred. Basic knowledge of the research base, theory, practices, principles, and processes of professional nursing. Interpersonal and communication skills to interact effectively with patients, families, and healthcare team members. Analytical ability sufficient to identify changes in patients’ conditions and to initiate appropriate action. Ability to identify potential crisis situations involving patients and/or families and to initiate appropriate action. Demonstrates a caring nursing practice and promotes a healing environment for patients and families. Demonstrates the required knowledge and technical skills to care for most patient populations on the unit.
