JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Participates in quality assessment performance improvement teams and activities. 18. Respectful of patient and family/caregiver environment and patient's personal needs. 19. As a mandatory reporter, reports failure to comply with the requirements of chapters 246- 335 WAC and 70.127 RCW to the Washington Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 20. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of RCW 74.34.020 or 26.44.030 to the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. POSITION QUALIFICATIONS 1. Completion of a state approved certified nursing assistant course or equivalent training 2. Current Healthcare Provider CPR (Cardiopulmonary Resuscitation) certification ((may not be internet based unless the demonstration of skills was hands on and observed by a certified trainer) 3. Due to nature of job and working with multiples facilities with multiple covid guidelines must be covid vaccinated. 4. High school diploma or General Equivalency Degree (GED) 5. Ability to read and follow written instructions and document care given. 6. Understands hospice philosophy, comfortable providing specialized care to the terminally ill. 7. Prior experience in hospice, orderly, acute , rehabilitative, or long term care facility. 1 year previous home health/hospice experience preferred 8. Basic computer skills to document in electronic medical records (EMR) 9. At least 18 years of age. 10. JOB SUMMARY The State Tested Nursing Aide provides personal care and related services in the home that are allowed to be performed under state law. She/he functions under the direction, instruction and supervision of the staff nurse and the Clinical Manager and/or appropriate supervisor. QUALIFICATIONS Prefer Completion of the 9th grade. Ability to read and write in English, consistent with job requirements. Must have a criminal background check and current CPR certification. Meets one of the following: A home health aide training and competency evaluation program as specified by CMS in the CoP § 484.80 (b) and (c) respectively, OR A competency evaluation program that meets CMS specifications, OR A nurse aide training and competency evaluation program approved by the state as meeting the requirements of 42 CFR 483.151-154, OR The requirements of a state licensure program that meets the provisions of CMS Home Health CoP § 484.80 (b) and (c). RESPONSIBILITIES/ESSENTIAL FUNCTIONS Tasks to be performed by an HHA must be assigned by and performed under the supervision of an RN who will be responsible for the patient care provided by the HHA. Perform services that are permitted under state law. Duties of the HHA may include: Assist with ADLS and iADLs as per the POC Follow the POC for each patient and document the same Help patients to maintain good personal hygiene Assist in maintaining a healthful, safe environment Meal preparation as per the plan of care Assist patients with ambulation as per the POC Assist with certain treatments as directed by the Skilled nurse Assist therapy personnel as needed with rehabilitative processes per the HHA POC May cue for medications ordinarily self-administered as assigned Taking and charting vital signs under the direction of a nurse or healthcare practitioner Encourages patients to be as independent as possible according to the nursing care plan Attempts to promote patient’s mental alertness through involvement in activities Gives simple emotional support to patients and other members of the household Establishes a relationship with patient and family which transmits trust and confidentiality Prepares a report of visits on the day it is performed and incorporates same in the clinical record weekly or as directed Reports any changes in the patient’s status or home situation immediately to the supervisor, staff nurse, or aide supervisor, including recognizing & reporting skin changes Carries out assignments as instructed by the nurse/Therapist or and documents and reports when she is unable to do so An active participant on the multidisciplinary team Works with personnel of other community agencies involved in the patient’s care as directed Performs routine housekeeping tasks as related to a safe and comfortable environment for the patient, as instructed by the nurse Attends in-service as required by state and federal regulation Timely & complete documentation per agency policies & procedures An active member of the care team, attending case conferences as required Confirming on a weekly basis, the scheduling of visits with the Supervisor/Director Notifying the Agency of absences due to illness, emergency leave, normal vacation periods, or special professional meetings, which will affect agreed service with the Agency Wears the Agency’s name badge at all times when seeing Agency patients Active Participation in the Agency QA Program Other duties as assigned SPECIAL EQUIPMENT TO BE OPERATED Vehicle to do routine patient visits, possible transfer/lifting devices, medical supplies WORK ENVIRONMENT Indoors, performing patient care/tasks in the patient’s residence Travel to patient residences/meetings; will require transportation to get to various work sites Possible exposure to communicable diseases FUNCTIONAL ABILITIES Able to access patient’s homes that may not be handicap accessible. Able to hear, speak, understand and communicate effectively in English. PHYSICAL REQUIREMENTS On a daily basis: walking, standing, sitting, reaching, use of telephone, use of computers & other office equipment, lifting/transferring 150 lbs. or greater, bending, stooping, climbing stairs.

Measure and record height, weight, and fluid intake/output. Recognize abnormal changes in body functioning and importance of reporting such changes to a supervisor. Assist as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living. Transfer, position, and turn residents. Provide skin care. Contribute to the resident’s assessment and the plan of care. Provide nursing rehabilitation/restorative nursing services. Care for residents with dementia. Follow the schedule of resident’s needs set out by supervisor. Provide emotional and social support to residents. Promote and protect resident rights, assist residents to make informed decisions, treat residents with dignity and respect, protect resident’s personal belongings, report suspected abuse or neglect, avoid the need for physical restraints in accordance with the current professional standards, and support independent expression, choice, and decision-making consistent with applicable law and regulation. Inform supervisor of any resident issues or concerns. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality. Familiar with emergency equipment and procedures. Understand duties during a fire drill and is familiar with evacuation plan. Attend all required training, in-service, and staff meetings. Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices. Maintain a positive and professional demeanor toward residents, visitors, families, and co-workers. Adhere to all policies and procedures of Senior Lifestyle Corporation. Perform other duties as assigned. Qualifications A High School Diploma or General Education Degree; or one to three months related experience and/or training; or equivalent combination of education and experience Experience working in a skilled, sub-scute, assisted living or memory care setting. CNA licensure if required by state. Basic computer skills. Ability to work as part of a team and handle multiple tasks safely and effectively. Proficient verbal and written communication skills.
