This is an Hourly position. Overview POSITION SUMMARY: The Advanced Practice Nurse will be responsible for: providing high quality patient care to the patients of OSF OnCall Urgent Care. Maintaining the consistency and quality of all services provided. Maintaining full and current patient medical records, including reports of all examinations, procedures, and other services performed by the provider or other support staff. Exercising diligence in assisting the OSF CEO in keeping costs of the practice to a minimum while maintaining a high level of quality care for patients. Providing a truly personalized, patient-centric experience to all patients of OSF OnCall Urgent Care. This position requires floating to various facilities. Utilize available internal and external resources to address patient needs, including behavioral health services, community-based programs, and other supportive care resources. Maintain all required active, unrestricted state licensure and certifications. Adhere to professional standards of practice, ethical conduct, patient confidentiality requirements, and organizational policies. Perform additional duties and responsibilities as assigned to support organizational goals and patient care excellence. Job Summary GENERAL SUMMARY We are seeking a highly skilled, compassionate Registered Nurse (RN) to deliver high-quality, patient-centered care through a combination of in-home patient visits, telehealth patient consults with Nurse Practitioners or Physicians as required, and administrative tasks. The CareConnect Team at WellBe supports timely response to patients’ changing needs and this role will be instrumental in delivering this care consistently and in a patient-first manner. This role primarily supports urgent/episodic interventions, working in partnership with physicians, nurse practitioners (APPs), and interdisciplinary care teams to address a range of patient needs across the care continuum. The RN will operate with a patient-first mindset, balancing strong clinical judgment with care coordination, education, and documentation responsibilities. The role has no direct reporting responsibility but will function as part of an integrated, interdisciplinary patient care team.
