Prepares and administers medications; changes dressings, applies bandages, removes sutures and other first aid procedures; uses CPR skills when necessary, all under physician supervision. Maintains supplies, equipment, stocks and sterilizes instruments; practices OSHA safety standards; performs accurate, legal, and ethical documentation at all times. Processes patient phone messages, returning calls and routing them to other team members as appropriate; calls patients to obtain and relay pertinent information for the physician. Upon physician approval and authorization, calls in prescriptions to the pharmacy. Uses communication skills with appropriate medical terminology; and follows appropriate legal and ethical professional conduct; authorized individuals will use the ChenMed Rx system to support physician medication dispensing. Provides health coaching to a defined group of patients to support healthy lifestyle choices. Follows up with coached patients via weekly calls. Maintains patient records and charts to ensure accurate record of treatment, including med profile, care plan, communication notes, etc. Provides instructions to patients/families for use of products and services utilizing nursing process to address learning needs. Prepares, administers, monitors, and discontinues IV fluids. Accesses IV delivery system (ie. peripheral IV start, implant cath, broviac, etc.). Administers medications and treatments as prescribed by the physician. Performs phlebotomy, laboratory specimen collection and point of care testing. Responsible for overall delivery of quality patient care that meets or exceeds established standards and provides assistance in activities to meet state and/or federal regulatory guidelines. Maintains safe, comfortable, and therapeutic environment for patients/families in accordance with established standards. Participates in Continuous Quality Improvement (CQI) Program. Provides supervisory visits for Home Health Aides and LVNs as required by state and federal regulations. Responsible for participating in on-call rotation and emergency call according to agency policy. Performs other duties as assigned.
