We are seeking an experienced Outcomes Nurse (RN) to support our Structural Heart Program , with a focus on Left Atrial Appendage Occlusion (LAAO) procedures, including WATCHMAN . This role is essential to maintaining compliance with CMS-mandated registry requirements , ensuring accurate data reporting, supporting program quality, and sustaining ongoing participation in advanced structural heart therapies and clinical initiatives. As part of a collaborative and growing Heart & Vascular Outcomes team, this position plays a key role in data integrity, longitudinal patient follow-up, and performance improvement efforts across the LAAO program. Department Overview Family Medicine at Scappoose is a primary care Family Medicine clinic and Rural Health Center (RHC). The patient population includes pregnant people, babies, children, adolescents, adults, and geriatrics. The patients are generally moderately stable to moderately unstable, with complex medical conditions and SDOH barriers that require medical and nursing interventions. These include, but are not limited to, oral and injectable medications, foot care, telephone and in-clinic triage, care coordination, health education, complex wound care, pain management, and behavioral and/or crisis management. Function/Duties of Position This position will cross-train to support additional primary care clinics within the Department of Family Medicine and the Division of Internal Medicine. Responsibilities include remote phone triage, MyChart message management, results review and follow-up, transitions of care, and other duties aligned with operational needs to support clinic workflows and the RN team. Job Description The perioperative registered nurse (RN) is responsible for the delivery of safe, effective, and quality patient-family centered care in the operating room for all patient populations. This primary position at Nemours Children’s Hospital, Surgery Center, Malvern, will include collaboration with Nemours Children’s Hospital, Delaware and the Nemours Children’s Hospital, Surgery Center, Deptford, to optimize available resources in achieving optimal patient outcomes. This position may require travel to these facilities. Travel is compensated based on organizational policy. This position is expected to maintain all state and federal licensure and clearances for PA, DE, and NJ for periodic rotation to the facilities in Delaware and New Jersey at the discretion of Perioperative Leadership. This includes active registered nurse (RN) licensure in Pennsylvania, Delaware, and New Jersey, and current Pennsylvania (PA) FBI Fingerprint Report, PA Criminal Background Record, and PA Child Abuse Clearance. This position requires working shifts aligned with the needs of the department and the assigned FTE of the RN. Schedule: full-time, 60 hours biweekly, Wednesday, Thursday and Friday 7:00 AM - 5:30 PM. No Weekends, No Holidays, No Call Responsibilities Provides age-specific, culturally competent, and ethical care within legal standards of practice. Assesses patient’s immediate pre-operative, intraoperative, and immediate post-operative status at intervals in compliance with organizational policies/procedures. Develops an individualized perioperative patient plan of care. Protects the patient from injury caused by extraneous objects and chemical, electrical, laser, mechanical, and thermal sources. Performs accounting procedures to protect the patient from unintended retained surgical items in compliance with organizational policies/procedures. Performs interventions necessary to ensure that the patient’s procedure is performed on the correct site, side, and level. Manages patient specimens according to organizational policy and procedures. Communicates the patient status to relevant parties throughout the continuum of care. Provides/administers medications safely and correctly in compliance with organizational policies/procedures. Performs interventions to maintain the patient’s wound and tissue perfusion at or above baseline level. Performs interventions to ensure the patient is at or returned to normothermia at the conclusion of the immediate postoperative period. Performs interventions to protect the patient from surgical site infection. Evaluates patient progress toward attaining outcomes. Qualifications PA RN License upon hire required Apply for DE RN License within 6 months from the date of hire. Apply for NJ RN licensure within 1 year from the date of hire.
