Wound Care Nurse (RN)

$30 - $35 / hour
Care Pavilion is a 396-bed acute and long-term care facility, located in the heart of Philadelphia and easily accessible via public transportation from all areas of the Philadelphia and the western suburbs. The Would Care Nurse, R.N . communicates and collaborates closely with the Director of Nursing, physicians, Unit Managers, and other members of the interdisciplinary team to provide consistent quality care to those patients and residents with wounds. They also work closely with the Director of Staff Development, Infection Prevention and Control Officer as well as the Quality Improvement team to ensure that all nursing staff is educated in the principles of wound prevention and treatment. The Wound Care Nurse implements treatment modalities as assigned. Responsibilities include but are not limited to: Assess skin condition of new admissions and readmissions using the standardized assessment tool Uses the Braden Scales to predict pressure sore risk as needed Implements treatment plan for patients and residents as appropriate and administers treatments Implements and evaluates the plan of care reflecting appropriate nursing and medical diagnosis, goal development, outcome, criteria and target dates. Keeps patient and resident plans of care updated reflecting changes in condition and responses to treatment or education Appropriately documents on patients and residents receiving treatments, reviewing previous documentation. In-services staff as needed in, including but not limited to, the following areas: Treatment modalities, pressure redistribution surfaces, positioning devices, nutrition, wound care documentation, classification of wounds, wound measurement, staging of wounds, and care planning. Registered Nurse Assessment Coordinator (RNAC)

$35 - $45 / hour
Care Pavilion Nursing and Rehabilitation is now hiring a full-time Registered Nurse Assessment Coordinator (RNAC). Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) are both eligible for this role. Apply today for an immediate interview! The Registered Nurse Assessment Coordinator is responsible for the timely completion of all OBRA and Medicare assessments to comply with federal regulations. Oversees the overall process and tracking of Prospective Payment System (PPS) Minimum Data Set (MDS) assessments including completion, submission and correction as needed. Job Responsibilities: Integrates nursing, dietitian, therapeutic recreation, restorative, rehabilitation, and physician services to ensure accurate assessments which capture all clinical conditions and provided services. Confirms payer source of all residents per HIQA. Ensures the accurate and timely completion of all MDS Assessments including OBRA and Medicare PPS. Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident’s stay. Responsible for electronic submission of all MDS assessments. Verifies electronic submission of MDS and performs corrections when necessary and maintains appropriate records. Coordinates interdisciplinary participation in completing the MDS for each resident according to regulatory time frames. Ensures completeness of MDS and thoroughness of documentation as mandated by federal and state standards. Schedules resident care conferences in compliance with state and federal regulations and ensures completion of all MDS reviews prior to resident care conference Assists disciplines with care planning process by ensuring the MDS information is completed timely, the Care Area Assessments (CAAs) are analyzed thoroughly, and the team has completed the MDS prior to care conference. Ensures all Medicare documentation follows federal and state guidelines and is completed timely. This includes the physician certification/Recertification, Traditional Medicare and Medicare Advantage non-coverage letters for part A and B. Assess and evaluates the outcomes of the RAI process to determine additional training, education and monitoring needs Validates that the documentation supports MDS coding. Collaborates with the RAI Specialist(s) and the Interdisciplinary team to identify and capture opportunities to assist residents in reaching their optimal physical and psychosocial levels. Job Qualifications: Education: Graduate of accredited school of nursing. Certificate/License: Current license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in PA. Work Experience: One year of clinical experience. Experience with MDS completion, reimbursement, clinical resource utilization is highly desirable.
