We are looking for a reliable LPN (Licensed Practical Nurse). The ideal candidate will be experienced in providing diverse basic care to those who need it while remaining patient and calm. It is a demanding job but also very rewarding.

Job Summary:
The Licensed Practical Nurse (LPN) is responsible for providing and coordinating nursing care in compliance with facility policies and procedures and to assess residents care needs and take appropriate action. The LPN is also responsible for the clinical direction and management of the STNA duties within the assigned nursing unit.

Responsibilities and major duties:
1. Ensure accurate and timely administration and distribution of all medications to assigned residents within facility.
2. Administer and document direct resident care, medications and treatments per physicians orders and accurately record all care provided. Follow established standards or nursing practices and implement facility policies and procedures.
3. Obtain report from and provide report to nurse being relieved or coming on duty and record/relay sufficient information to implement appropriate follow-up action as necessary.
4. Collect data and properly document comprehensive assessment of residents as assigned.
5. Develop and implement an accurate comprehensive care plan based on each resident’s needs and his/her comprehensive assessment.
6. Identify resident problems and emergency situation and initiate emergency care and “life-saving” measures in the absence of a physician.
7. Provide supervision to Nursing Assistant providing direct resident care.
8. Counsel Nursing Assistants and recommend disciplinary action to the Director of Nursing Service, when necessary.
9. Participate in the employee evaluation process by providing information on the performance of Nursing Assistants when requested.
10. Promptly report changes in residents conditions to the physician, Director of Nursing Service and responsible party, and take follow-up action as necessary.
11. Order, receive and store medications appropriately.
12. Document resident progress notes as required.
13. Consistently work cooperatively with residents, nursing staff, physicians, families, consultant personnel, community agencies and ancillary service providers.
14. Obtain continuing education required to maintain licensure.
15. Observe infection control and safety procedures.
16. Attend and participate in all scheduled staff meetings, educational meetings/in-services and all other meetings related to employment.
17. Assist with and participate in all resident activities to facilitate a home like environment for the residents within assigned facility.
18. Ensure compliance with all HIPAA regulations.
19. Respectfully engage and welcome all visitors, guests and family members within the facility.
20. Ensure a safe and sanitary living environment for residents.
21. Adhere to all infection control policies within the assigned facility.
22. Ensure the Resident Assessment Instrument (RAI) process is followed in accordance to State & Federal Guidelines.
23. Follow Resident Rights policies at all times.
24. Adhere to all facility policies and procedures.
25. Ensure adherence to the Corporate Integrity Program and the Performance Expectations at all times.
26. Perform other related duties as directed by Licensed Nurse and/or person of authority.


Position Qualifications

Education and Experience: Graduate of an accredited Practical Nurse or Vocational Nursing Program. 1 year of LPN experience preferred. Ability to add and subtract and perform basic mathematics as it relates to the position is required. Ability to read/write/comprehend the English language in written & verbal form required. Ability to communicate both verbally and non-verbally required.

Licensure, Registration and/or Certification: Successfully complete &/or comply with all required background checks to include but not limited to: Ohio BCI&I fingerprint analysis, FBI (if applicable) fingerprint analysis, Nurse Aide Registry, HHS/OIG list of Excluded Individuals/Entities (LEIE) & Ohio Medicaid Provider Exclusion and Suspension List. Valid License to practice as a LPN in the State of Ohio. CPR certification required.

Responsibility for Patient Care: Provide services based on physician orders. Communicate physician, patient and family needs with the appropriate caregivers.

Computer: Intermediate personal computer skills including electronic mail, routine database activity and word processing.

Leadership Responsibilities: Responsible for the direction and management of the STNA’s and/or Restorative Aides within the facility.

Responsibility for confidential matters:
Works regularly with some information such as medical records or reports, financial reports, information on computer screens or other electronic devices, and records that are restricted to specific persons. Must comply with confidentiality policy.

May be exposed to heat and cold in all weather conditions. Works regularly under desirable conditions with little or no disagreeable features.   Distractions are frequent.
