We have immediate openings for Home Care RN’s to serve multiple locations in Lorain County. We have both full and part time positions available for RNs. All positions are paid on a per visit basis at $45 per visit/point. Travel is paid per mile. Weekend every 3-4 weeks. 1-2 Holiday a year.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
