All positions are paid on a per visit basis at $45 per visit point. Travel is paid per mile. Weekend on call every 3-4 weeks. 1-2 Holiday a year.

Complete, coordinate, and revise patient Plans of Care in collaboration with the care team. Maintain accurate, timely, and compliant clinical documentation. Communicate changes in pain/symptom management. Educate families on patient condition, needs, and care expectations. Make referrals to other hospice disciplines as needed and document rationale. Deliver on-call responsibilities and services as assigned. Perform other related duties as needed. Requirements Must be a Registered Nurse with hospice experience preferred; telephone triage experience a plus. Active RN license in good standing with the State of Ohio. Strong understanding of hospice philosophy and the unique needs of terminally ill patients. Flexible availability for after-hours, weekend, and on-call schedules. Strong computer skills and ability to navigate EMR and software systems. Ability to handle stressful situations and support grieving families.
