LPN 1:1 School-Based Support (Elementary)

Were looking to connect with a compassionate Licensed Practical Nurse (LPN) for a 1:1 role supporting an elementary student. This is a meaningful opportunity to provide consistent care and build a supportive, trusting connection throughout the school day.

Position Details

  • 1:1 support for a female student
  • Bus riding required: pick-up at 9:00 AM, drop-off at 4:21 PM
  • The student uses an adaptive stroller; lifting support is required
  • Full assistance needed for diapering using a height-adjustable changing table (two-person transfers required)
  • Non-verbal; uses eye-gaze AAC (not yet fully functional)
  • Medical needs include G-tube feeding and tracheostomy care

Responsibilities

  • Provide full 1:1 medical and personal care support throughout the day
  • Assist with mobility and safe transfers
  • Support with feeding, airway management, and daily care routines
  • Monitor communication needs and ensure student comfort and safety
  • Collaborate with school staff to support the students day-to-day needs

Clinical Requirements

  • Active LPN license
  • Experience with G-tube and tracheostomy care required
  • Comfortable with pediatric and school-based care needs
