Admissions Nurse (RN)

Elderwood of Lakeside at Brockport, a Skilled Nursing Facility, is seeking a Full-Time Admission Nurse! This is an exciting opportunity for a Registered Nurse to coordinate and facilitate all aspects of resident admissions to the facility. They will ensure a smooth transition of incoming residents by completing clinical assessments, verifying appropriate room placement, and collaborating with interdisciplinary teams to meet regulatory and resident standards. The Admission Nurse partners closely with our Director of Nursing and Administrator to drive quality initiatives, support staff development, and uphold regulatory excellence - all within a community known for teamwork, respect, and clinical pride.
On days when there are no admissions, the Admission Nurse will be available to help on the floor, assisting with medication passes, resident discharges, treatments, and providing overall assistance to the nursing department.
Schedule:
Monday-Friday 11am-7pm with weekend on-call rotation
Responsibilities
Key Responsibilities:
- Complete clinical assessments upon admission
- Initiate care plan in PCC and complete Nursing section
- Place new orders in PCC
- Verify diagnoses, medications, treatment orders, etc.
- Ensure accuracy of orders and identify immediate care needs
- Communicate with hospital prior to admission to obtain clinical report
- Communicate facility procedures with families and residents
- Assume responsibility for nursing services in the absence of the Assistant Director of Nursing Services and/or Director of Nursing Services.
Skills/Competencies:
- Valid RN license in New York State required
- Effective problem-solving skills
- Experience with Point Click Care (PCC) preferred but not required
- Caring, compassionate and empathetic attitude
- Able to relate well with residents, families and co-workers
- Exceptional customer service
- Excellent communication skills both verbal and written
- Team-focused demeanor
- Strong supervisory skills for managing and coordinating care of residents

Qualifications
Admission Nurse (RN):
- Possession of a state license in applicable state you are performing work and current registration to practice as a Registered Professional Nurse
- Minimum of two (2) years of nursing experience and at least one (1) year of experience in a nursing leadership role
- Interest or experience in geriatrics
- Current BLS/CPR
- Experience with dementia population desirable
- This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
