Under the direction of the ATC Practice Manager and guided by the policies and procedures of Ambulatory Services, the Registered Nurse (RN) provides care to uncomplicated and high-risk OB patients in the Antenatal Testing Center, UCSF Department of Women’s Health. The RN will play a critical role in the patient experience, including optimizing clinic flow, providing in-person and outreach to patients, and working collaboratively with the team to create a patient-centered environment.

I n collaboration with the ATC Medical Director and Nurse Supervisor, the nurse has the clinical responsibility for maintaining a safe environment for patients and staff, and for daily operations. The RN is a team player who works closely with others and is flexible in handling changing priorities. They are self-reliant individuals who synthesize their knowledge of practice operations to problem-solve, prioritize, and facilitate complex transactions in the course of their daily activities. They function in an expanded nursing role in the administration and interpretation of fetal surveillance tests. All components of nursing care will be individualized to meet the patient’s physiological and psychosocial developmental level and needs.

The RN has excellent oral and written communication skills; can work well with minimal supervision in a complex, fast-paced, and challenging environment. S/he is sensitive to the needs of patients, staff, and providers at all times. This position makes a difference for patients in an outpatient care unit by providing excellent nursing care and customer service, facilitating and ensuring the accuracy of the information flow between medical, hospital staff, and departments to maximize unit efficiency. S/he consistently projects a professional and courteous demeanor and shall comply with the Medical Center House and Telephone Standards. This position requires the flexibility to orient and work at all UCSF Medical Center locations.

Responsibilities

DUTIES & ESSENTIAL JOB FUNCTIONS

Assessment

  • Identifies the needs of patients through observation of disease symptoms, Emotional state, laboratory reports, cultural and socioeconomic background and response to medical regimen and treatment.Uses knowledge of the cognitive, physical, emotional and chronological maturation process.
  • Responds or refers inquiries appropriately.Recognizes and reports specific physical and emotional conditions/needs/concerns of the patients to the appropriate members of the health care team.Alerts physician of unusual observations of patients to facilitate immediate attention, e.g. chest pain, shortness of breath.

Planning

  • Works cooperatively and participates with the health care team to discuss patient’s alternatives to health care services and to develop short- and long-range goals to meet the patient’s needs.
  • Plans as appropriate for the needs of patient using knowledge of physical and emotional processes as well as cultural and socioeconomic backgrounds.
  • Identifies appropriate resources (e.g. family, significant others, monetary, community organizations) in planning for patient care.

Implementation /Evaluation

  • Provides individualized nursing care which responds to the growth and developmental needs of patients and demonstrates an understanding of the range of treatment required by the patient.
  • Demonstrates ability to provide appropriate care based on the needs of a specific individual, including that individual’s age – Adolescent, Adult (19-64 years).
  • Prepares for and instruct patients about specific diagnostic procedures (e.g. non stress tests, ultrasounds, etc.)
  • Performs relevant nursing activities as indicated including but not limited to: fetal/uterine monitoring,

limited OB ultrasounds, biophysical profiles, cervical exams and membrane sweeping.

  • Maintains in depth knowledge of the Guidelines for fetal Surveillance, including the tests, indications and frequency.
  • Displays excellent clinical judgement in the interpretation of fetal heart rate monitoring, limited OB ultrasounds and Biophysical Profiles as evidenced by the approval of the ATC Nurse Manager and ATC Medical Director.
  • Maintains knowledge and skill with the operation and care of biomedical equipment in the practice.
  • Evaluates and documents patients’ responses to nursing care.Alerts physician of unusual observations of patients to facilitate immediate attention.
  • Responds to the emotional needs of patients by demonstrating sensitivity to their emotional state by relating to patients in an appropriate manner.Refers to appropriate members of the health team as indicated.
  • Responds appropriately in emergencies including fetal distress.Is knowledgeable about the emergency supplies available on site and can initiate and assist in CPR. Understands the procedures for medical emergencies, code blue and other emergency/disaster situations.
  • Complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, administrative and clinical practice.Complies with requirements/standards by TJC/Title 22 /NCQA.
  • Completes annual competency testing as specified by the individual practice.

Coordination/Continuity/Communication

  • Organizes nursing care activities based on the needs of patients while maintaining the effective flow of patients through the ATC.Adjusts the clinical support activities to reflect changes in priorities during practice sessions.
  • Maintains open communication and facilitates continuity of care for patients and/or family with members of the health care team through phone contact, verbal interaction and/or written reports.Utilizes APeX and other UCSF information systems to contribute to patient care.
  • Maintains accurate, legible, current, properly signed records and documentation which include nursing observations, interventions and care plan, nursing actions/teaching performed.
  • Obtains, records, and conveys information, clearly and concisely.
  • Participates in patient-centered conferences as an integral member of the patient care team; participates in family conferences when appropriate.
  • Coordinates diagnostic tests and labs and participates in tracking follow-up results and communications from others providing care to the patient.
  • Maintains open communication with ancillary departments such as radiology/ultrasound, pharmacy, sterile processing, and the laboratory to problem-solve issues.
  • Demonstrates courtesy, common sense and follow-through in all telephone communications, maintaining the Telephone Standards as established by the Medical Center staff.
  • Fosters/maintains positive public relations (patients, family, visitors, and staff) Uses effective communication skills with patients and staff; demonstrates proper telephone techniques and etiquette; shows sensitivity to differences of culture; demonstrates a positive and supportive manner in which patients perceive interactions as positive and supportive.

Advocacy/Education

  • Participates in identifying the educational needs of patients.
  • Provides patients with the necessary information to make informed decisions utilizing available resources and making referrals appropriately. Informs patients/families about services available at UCSF and in the community.Utilizes and maintains updated educational resources.
  • Provides and reinforces patient health teaching by utilizing language which patient and family can understand.
  • Verifies patient/family understanding through return demonstration - either verbal, physical, or written.
  • Utilizes available educational resources.Participates in review and/or revision of educational materials as needed.

Performance Improvement

  • Participates in Performance Improvement (PI) activities.
  • Makes recommendations regarding PI topics or problems; assists in data collection as requested.
  • Reports and follows up on issues that effect environment and safety.
  • Maximizes resources by seeking assistance from appropriate support, effectively using supplies, making suggestions for improved approaches, assists in cost effective practices.

Operations Maintenance

  • Monitors POCT and assures QI is completed as required.
  • Actively participates in preparation for surveys (TJC/CLIA/DHS).
  • Oversees maintenance of adequate levels of clinical supplies, laundry, and other items necessary for optimal practice.

OTHER FUNCTIONS AND RESPONSIBILITIES

Demonstrates innovation and flexibility regarding the modification of tasks or workflows within assigned duties and job functions.

Performs other duties as assigned. Can include but not limited to being cross trained for other Women’s Health services.

Consistent with training, UCSF Medical Center Policies, and the TUGs Operations Manual, receives TUGs; removes, distributes and/or places contents in appropriate storage areas; cleans and disinfects TUGs; deploys TUGs; returns TUGs to appropriate holding areas.

Has an excellent attendance record.

Qualifications

Required Qualifications:

  • Current Antenatal Testing Center experience, including demonstrated proficiency in the use of real-time ultrasound, or two

years of Labor and Delivery experience (within the last 2 years) caring for both routine and complex patients

  • Active licensure as a Registered Nurse in the State of California
  • Active American Heart Association CPR Certification
  • Knowledge and abilities essential to the successful performance of the duties assigned to the position
  • Excellent communication skills
