LPN School Contract

Key Qualifications:

  • Active LPN or LVN license required
  • Direct experience with tracheostomy care and g-tube management
  • Previous pediatric, school, or 1:1 healthcare experience is preferred
  • Compassionate attitude, reliability, and effective communication skills
  • Ability to collaborate with school staff and home health nurses

Primary Responsibilities:

  • Deliver daily 1:1 care to the student during school hours and bus transportation
  • Manage diapering, trach care, trach suctioning, and g-tube care as needed
  • Accompany the student to and from school, meeting at the school and riding the bus to the student’s home
  • Establish rapport with the student and family, working alongside the home health nurse for smooth transitions
  • Maintain accurate documentation of care and communicate any changes in health status to appropriate parties
