Job Description

Under the direction of the Ward 86 Nurse Manager, the Registered Nurse (RN) will work in a primary care setting, delivering comprehensive nursing care within a multi-disciplinary team structure. Responsibilities include, but are not limited to, providing direct patient care, administering medications, coordinating patient care, conducting assessments, and managing sexually transmitted diseases. The RN will also provide patient education and actively participate in quality improvement initiatives related to triage and other programs as required. Attendance at relevant meetings will be expected as part of this role.

About Ward 86 Clinic

Ward 86, the HIV/AIDS Clinic at the Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG) campus has been a leader and innovator in field of HIV care since the beginning of the epidemic. Ward 86 is the largest clinic providing primary medical care for people living with HIV for the San Francisco Health Network (SFHN) and also provide preventive services to patients at-risk for HIV infection, including pre-exposure prophylaxis (PrEP). Ward 86 provides care to approximately 2500 patients living with HIV, with over 24,000 clinical encounters a year. Services are offered through a comprehensive, multi-disciplinary approach, addressing primary medical and HIV care, urgent care, mental health evaluation and treatment, substance use assessment, treatment and referrals, HIV prevention services, and HIV specialty care needs (including on-site Pulmonary, Dermatology and Cardiology clinics), and psycho-social assessment and interventions.

The Division of HIV, Infectious Diseases and Global Medicine at the University of California, San Francisco (UCSF) is an internationally recognized global leader in clinical care, research, and education. Primary Care Nursing Care Responsibilities

  • Organize and provide nursing care to a patient population living with, or at risk of contracting, HIV/AIDS in an outpatient primary care setting.
  • Assist patients with navigating healthcare systems, including coordinating care with various specialty clinics and outside agencies (e.g., pharmacies, social services, community based programs, and so on).
  • Serve as patient advocate to ensure comprehensive care is delivered, including serving as an intermediary to facilitate problem resolution within an interdisciplinary setting.
  • Facilitate patient medication management, including administration, providing patient education around treatment adherence, interacting with pharmacies and insurance providers in collaboration with providers and pharmacy team.
  • Assess patients, analyze findings, plan nursing care, implement interventions, and evaluate care and outcomes. In addition to traditional nursing care, this includes, but is not limited to, screening patients for sexually transmitted infections, assessing risk factors, and implementing interventions as indicated; planning and executing interventions to engage patients as partners in care; conducting various screenings of potential health problems and, in collaboration with interdisciplinary team, implement interventions when problems are identified.
  • Act as a central member of the interdisciplinary team and actively partake in the management of patients with multiple comorbid conditions and patients who are at high risk of adverse outcomes. This includes, for example, identification of barriers to care, developing strategies to address identified barriers, and outreach to and planning for patients who have had recent hospitalizations.
  • Collaborate closely with clinic staff and ac as a vital member of the nursing team at the clinic.

Direct Nursing Care Responsibilities

  • Organize and provide direct nursing care of patients seeking care in clinic and via EPIC patient calls and advice requests. This includes assessing, analyzing, planning, executing, and evaluating nursing care of primary care patients coming in for urgent problems. This includes prioritizing presenting problems, implementing interventions, and managing care as a member of the multidisciplinary team. This includes using traditional bedside and emergency/acute care nursing skills.
  • Organize and oversee transfer of patients to higher levels of care if needed, such as to the Emergency Department (ED), an urgent care clinic, or to inpatient units.
  • Provide direct patient care for urgent care and primary care patients (traditional bedside and emergency/acute care nursing which includes but is not limited to peripheral IV insertions, phlebotomy, medication administration, IV fluid administration, etc.)
  • Administer long acting injectable medications to patients.
  • Support triage workflows for medical referrals related to patient care plans.

Qualifications

Required Qualifications

  • One year of clinical experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position.
  • Experience working with patients living with HIV/AIDS.
  • Experience working with patients with complex mental health conditions, including substance use disorders.
  • Experience working with patients experiencing homelessness or unstable housing.
  • Experience working with patients requiring STI management.
  • Experience with administration of long acting injectable medications.

Preferred Qualifications

  • Fluency in Spanish.
  • Bachelor of Science in Nursing (BSN) or higher.
  • HIV/AIDS Certified Registered Nurse (ACRN) Certification.
  • Two (2) years experience as a Registered Nurse in a primary care setting.

Required Licenses And/or Certifications

  • CA Registered Nurse (RN) License.
  • Graduation from an accredited school of nursing.
  • AHA Cardiopulmonary Resuscitation (CPR) Certification.
