
Nurse Practitioner/Physician Assistant - PACE of the Ozarks

Position Summary
The role of APRN/PA is responsible for providing direct patient care, assisting physicians with procedures, and assisting with other duties, as needed.
Essential Position Responsibilities
- Perform age-appropriate history and physicals for PACE population.
- Orders, interprets diagnostic tests relative to patient's age-specific need and co-morbidities.
- Implement interventions to support patient stability.
- Actively participates in Interdisciplinary Team regarding care.
- Completes assessments as per PACE Regulations Documents services in the Electronic Medical Record in a timely manner.
- Serves rotation of On-Call status.
- Provides direct patient care, diagnosing, and treating health problems and coordinating health care for PACE population in accordance with State and Federal rules and regulations.
- Participates in the overall development of Plans of Care for PACE population.
- Skin and Wound Care
- Prescribes appropriate treatment modalities, performs medication reconciliation on PACE population.
- Serves as a Liaison with specialists in the community.
Qualifications
- Education: Must be a graduate of an accredited Family Nurse Practitioner or Physician Assistant program
- Licensure and Certifications: Nurse Practitioner or Physician Assistant License and CPR, required. National certification, required.
- Experience: Minimum 3 years’ experience in a previous geriatric clinical nursing role, required.
Work Environment: This position will spend 80% of time standing and/or walking while pushing, pulling, lifting, and/or carrying up to 50lbs. This position will spend 20% of time sitting while performing work in a standard office environment.
