 The Hospice Team Nurse provides professional nursing care to patients by utilizing all elements of nursing process.

Job Description The Sr Licensed Vocational Nurse, under the supervision of the Nurse Manager, guided by the procedures of the Ambulatory Services and the Ambulatory Services Quality Nursing Division, and in collaboration with the physician staff, had the clinical responsibility for developing and maintaining a high standard of patient care, under the Urology Procedure Center and Urology Faculty Practice. They participate in the planning for the provision of patient care. They provide patient health education teaching for Urology patients across the lifespan, catheter teaching and management skills and clinical support. They collaborate with the Nurse Manager, Administrative Director, and Medical Director to develop monitor and maintain systems pertaining to Urology patients. They are responsible for maintaining a safe environment for patients and staff, and for daily operations. All components of nursing care will be individualized to meet he patient’s physiological and psychosocial developmental level and needs. The Licensed Vocational Nurse consistently projects a professional and courteous demeanor and shall comply with the Medical Center House and Telephone Standards. The Sr Licensed Vocational Nurse will be primarily located at Bayfront, however, must have flexibility to cover multiple UCSF Medical Center sites. Job Description JOB SUMMARY The LVN utilizes the nursing process to assess and identify the individual needs of patients through observation of disease symptoms, emotional state, laboratory reports, cultural and socio economic background and response to medical regimen and treatment.
