RESPONSIBILITIES

I. JOB SUMMARY:

• Under the supervision and the direction of a registered nurse, physical, occupational or speech therapist in the community, assists in the care of patients in the patient’s home following specific care plans.

II. TYPICAL PHYSICAL DEMANDS:

A. ESSENTIAL FUNCTIONS:

• Seeing. Hearing. Speaking. Finger dexterity.

B. MANUAL MATERIAL HANDLING:

• Infrequent: N/A

• Occasional: Lift waist to shoulder between 36-50 lbs. Push and Pull between 36-50 lbs.

C. NON-MANUAL MATERIAL HANDLING:

• Infrequent: N/A

• Frequent: Stand. Sit. Walk. Stoop/Bend. Twist. Gripping.

• Constant: Reach: shoulder level. Reach: above head.

III. TYPICAL WORKING CONDITIONS:

• Performs work primarily in patients' homes and community settings with varying environmental conditions.

• May be exposed to communicable diseases, bloodborne pathogens, pets, tobacco smoke, adverse weather, and other household or environmental hazards.

• Frequent travel between patient homes and assigned locations is required.

• Uses appropriate infection prevention and safety practices, including personal protective equipment (PPE), in accordance with organizational policies and regulatory requirements.

IV. MINIMUM QUALIFICATIONS:

A. EDUCATION:

• High School diploma or equivalent.

B. CERTIFICATION AND LICENSURE:

• Certificate of Nurse Aide Training.

• Valid U.S. driver’s license, vehicle and vehicle insurance as required by Hawaii State Law.

• Driver's abstract reflecting evidence of good driving record.

• Current BLS certification.

C. EXPERIENCE:

• Knowledge of occupational therapy theory and practice to treat patients.

• Knowledge and use of medical equipment and instruments to administer patient care.

• Skill in identifying problems and recommending solutions.

• Skill in establishing and maintaining effective working relationships with clients, co-workers, and the public.

• Knowledge of Home Health Aide theory and practice to give and evaluate patient care.

• Knowledge of medical equipment instruments to administer patient care.

• Able to apply principles, methods, techniques, policies, and procedures necessary for a Home Health Aide.

• Able to complete electronic records and respond to clinical disciplinary team promptly to provide quality care for the patients.

• Able to establish and maintain effective relationships with patients, health care providers and the public.
