Job Summary:

We are seeking a skilled and detail-oriented RN MDS Nurse to join our interdisciplinary team. As an MDS Nurse, you will be responsible for the federally mandated assessments and will work closely with the interdisciplinary team to create applicable care plans for our residents. If you have an eye for detail, take pride in producing accurate and reliable assessments, and are knowledgeable about MDS 3.0, CAA's, Medicare Guidelines, and Care Planning, we encourage you to apply.

Responsibilities:

  • Gather information on facility residents for assessments

  • Work within the time frames set by Federal and State Regulations for all MDS sections

  • Assess charts and assist with communicating with IDT to create applicable care plans for residents

  • Prepare and track Medicare certifications and recertifications

Qualifications:

  • Current NYS RN license

  • Knowledgeable about MDS 3.0, CAA's, Medicare Guidelines, and Care Planning

  • Excellent written and oral communication, including English usage, grammar, and punctuation

  • Ability to work independently and work under pressure with time constraints

Compensation:

  • Salary: $32 - $38 HOURLY
