Provides patient care educational information and training to patient and family members. May serve as a resource for specific clinical patient services. Maintains and implements knowledge of applicable rules, regulations, policies, laws and guidelines that impact or govern nursing practice, and ensures the hospital and Parkland are in compliance. This includes EMTALA, Order of Productive Custody, Memorandum of Transfer and APOWW. Develops effective internal controls that promote adherence to applicable state/federal laws and the program requirements of accreditation agencies and federal, state, and private health plans. Maintains and updates knowledge of sanitation, infection control, safety, supplies, equipment, appropriate utilization of supplies and materials. Completes appropriate patient assessments, admission criteria, identification and implementation of appropriate clinical interventions, discharge planning, interdisciplinary communication. Utilizes critical thinking skills and recognizes emergency situations and provides appropriate nursing interventions. Completes appropriate documentation by maintaining accurate and timely entries and notations in patient care records and hospital documents. Ensures that all records and documents meet the standards, goals, and objectives of the assigned Surgical department and Parkland. Seeks and utilizes direction from more experienced staff and team members as needed. May work with other staff in providing direction to non-nursing staff. Participates in information exchanges with nursing and other Parkland staff that support the standards, missions, goals, and objectives of the assigned Surgical department and Parkland. May serve as a preceptor or trainer to newer nursing staff members. May serve as a Charge Nurse on rotation as necessary. Identifies ways to improve work process and improve patient and customer satisfaction. Makes recommendations to supervisor, implements and monitors results as appropriate in support of the overall goals of the assigned Surgical department and Parkland. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Clinic Nurse (CMA or LPN) - Mattoon Walk-in Clinic PART-TIME Job Description Clinic Nurses are responsible for providing assistance to providers in the direct and indirect care of Practice Management patients. Performs patient or office related duties including but not limited to clinical measurements and data collection. Documents data and provider directed information clearly and accurately in patient records. Functions clinically as support staff to provider(s). Location: Mattoon Walk-in Clinic Hours: Part-time/20 hours per week Required: HS Diploma Pay: Based on experience, starting at $20.59/CMA and $23.87/LPN Responsibilities Accepts reassignment, at the direction and discretion of management, as applicable to staffing levels and patient volumes., Administers medication as directed by the provider., Assists providers with patient exams/ treatments/procedures., Cleans/stocks exam rooms/work spaces on an on-going basis to assure that there is an adequate supply. Performs all tasks related to stocking and cleaning in adherence to regulatory and system infection control policies/protocols. This includes responsibility for discarding outdated items and labeling multi-dose vials with date and initial opened and then discarding them per guidelines., Cleans and sterilizes equipment and instruments based on needs of the practice. Performs tasks and documents them in alignment with infection control regulations and system policies/protocols., Collects patient data including but not limited to heights, weights, vital signs and other measurements., Documents appropriate data/information. Keeps providers aware of patient requests, needs and observations made that could be of value to the provider's assessment, care and follow-up., Documents responses to patient history/screening questions for use by providers during assessments and examinations., Follows Universal Precautions at all times during contact with patients, equipment, instruments, and/or specimens., Obtains provider initials or signature on phone message communications to verify accuracy of information communicated., Performs and documents laboratory duties which may include phlebotomy, specimen collection and performance of routine basic waived testing. Handles and coordinates reference lab testing specimens/reports and corresponding reports/documentation., Performs general clerical duties, as required, such as receiving and mailing patient test results, preparing charge tickets, transcribing data to physical or referral forms, filing and monitoring or ordering supplies., Relays information between provider(s) and client(s), which may include but is not limited to communications with patients, families, pharmacies, nursing homes, consultancy office staff, insurers, and facilities. At provider's instruction, communicates provider advice, instructions, orders and results, as applicable, including accurate documentation., Schedules patients for diagnostic, therapeutic, referral or additional office visits.
