Job Summary : To assist in the delivery of health care and patient care management in a clinical capacity. Daily responsibilities of the position are based solely on the needs of the practice at any given time. Job Description Essential Responsibilities: 1. Provides directly and through delegation patient care for facility clients. a. Maintains knowledge of normal/abnormal parameters that identify clients at risk. b. Provides patient care in accordance with standards and guidelines. c. Responds to telephone calls and triages appropriately. d. Provides patient with reassurance, accurate teaching and direction. 2. Ensures efficient delivery of healthcare services throughout the duration of the patient visit. a. Ensures that medical records are completed, accurate and updated. b. Initiates patient contact and prepares patient for examination. Obtains vital signs, weight, age, temperature, blood pressure, pulse, respiratory, history and chief complaint. c. Observes patients with abnormal signs and symptoms, communicates their condition to the physician and documents the same in the medical record. d. Ensures patient understanding of physician instruction upon discharge. e. Documents patients medical record entries and documents charge information into electronic medical record for billing. 3. Assist with testing and treatment procedures under the Physicians supervision. a. Assists physician in applying splints and dressings. b. Performs and follows through in all indicated orders from physician. c. Assists with sample medications and sample medication logs. d. Arranges and administers any necessary patient testing or admission. Obtains appropriate referrals required. Retrieves test results as needed and notifies patient. e. Assists physician in preparing for minor surgeries and physicals. f. Obtains appropriate consents before all invasive procedures or immunizations are performed. g. Distributes patient education materials as directed. Assists in patient instruction and ascertains patient understanding regarding treatment and medication usage. h. Performs laboratory tests and treatments as prescribed. i. Assists in maintaining laboratory log. 4. Ensures all patient care areas, equipment and materials are appropriately stocked and maintained. a. Maintains and organizes adequate supply of patient education materials. b. Orders and maintains required supplies and equipment for clinical care areas ensuring exam rooms are stocked appropriately. c. Keep exam rooms clean, free of waste and organized in accordance with cleaning protocol. Ensures that instruments are sterile and equipment is in working order. 5. JOB RESPONSIBILITIES: Completes the registration of patients into the hospital computer system according to hospital procedures, including: Performs bedside registration for Emergency Room patients. Pre-registers and registers patients for direct admission, scheduled admissions, outpatient surgeries and outpatient ancillary services in order to allow timely and accurate billing to third party payers and patients. Verifies and assures accurate documentation of financial and demographic information. Updates computer system if needed and makes copies of insurance cards for appropriate departments. Explains purpose of and obtains signatures for all consent forms and witnesses signatures of patients and responsible parties (consent for treatment, financial consent, Medicare Message advance directive, MSP form, and the HIPAA Revalidation consent form). Scans all forms for consent to treatment, financial forms, insurance cards, Photo ID's, HIPAA revalidation forms, ABN forms, and etc. Enters PA # and pre-cert number from insurance certification/referral letters-scans letters into system. Scans the PATA orders into system, plus some off-site registrations into system. Obtains pre-certification, referrals and prior authorizations on all hospital admissions, ER managed care, same day surgeries and required ancillary services within 24 hours of admission or next working day. Keys all required certification information into hospital computer systems. Coordinates with Case Management Department to ensure accurate execution of third party benefit program activities to best benefit the patient. Coordinates with Patient Access Specialists to provide accurate information to facilitate verification of benefits. Collects co-pays/deductibles for ED services. Also functions as Cashier during hours when the Cashier's office is closed. Accepts payment for admission deposits and patient valuables and accepts payment for current and past medical services and issues a receipt to the patient. Assists with coverage of Hospital Switchboard, responds to codes, carries responding phone on person when no dedicated switchboard operator is on duty. Performs related miscellaneous duties as assigned by supervisor. Conducts all activities in compliance with applicable laws, regulations, standards and hospital policies and procedures including Blood and Body Substance Precautions. Escorts patients to assigned nurses’ station or directs to ancillary department upon completion of registration process. EDUCATION, EXPERIENCE AND TRAINING: High school diploma or equivalent required. Previous CRT experience preferred. Previous experience in hospital admitting department or previous clerical experience in a medical setting preferred.
