The CLA assists in creating a nurturing supportive environment while helping participants (residents) achieve maximum self-reliance and independence within the community living setting. The CLA performs the care or services that are outlined in the individual participant’s care plan; any care guidelines that are pertinent to the participant’s diagnosis are specified in the care plan. Help clients with grooming and dressing, ensuring their personal care needs are met. Remind clients to take self-administered medication (medications they take on their own, with reminders from you). Observe and promptly report any changes in a client’s physical, mental, or emotional condition to your supervisor. Build and maintain professional relationships with clients, their families, and co-workers. Participate in ongoing training, staff meetings, and client discussions as needed. Complete accurate documentation of the services you provide. Safely use all equipment and supplies. Protect the confidentiality of all client information. Perform any additional related duties as assigned. Qualifications: 6 Months of personal or professional experience. Reliable transportation to and from work.
