Encore Village of Schaumburg is looking for a dedicated Restorative Aide to support residents in maintaining and improving their functional abilities. RESPONSIBILITIES: · Performs restorative treatments under the direction of the Director of Nursing · Collaborates with nursing staff to ensure that residents are available for the restorative programs, when necessary, will assist the nursing staff with direct care. · Documents all restorative activity performed in EMR. Completes and maintains documentation according to Rehabilitation Restorative program guidelines and facility policy. · Reports and documents residents’ significant changes to assigned CNA, Nurse and Restorative Nurse. · Updates and maintain quarterly restorative progress notes for the restorative nurse and rehab nurse. · Consults with unit staff routinely to be aware of resident diagnosis, precautions, and contraindications to performance of various restorative treatments. · Recognizes and treats emergency situations in accordance with facility emergency policies. · Transports residents, equipment, and supplies necessary to perform treatment. · Follows established procedures and safety guidelines pertaining to the care and use of equipment and supplies Gets training on new equipment prior to usage. · Prepares, maintains, and cleans treatment areas in preparation for direct patient care. Reports any equipment that is need of repair to Director of Nursing. · Recognizes the importance of providing Resident Centered Care. Regulatory Compliance · Respects Residents Rights for Privacy and Dignity · Reports all suspected, alleged, and/or witnessed verbal, physical, or sexual abuse immediately to supervisor.(PHI) Leadership · Serves as a role model and instructor for C.N.A.’s in demonstrating proper rehabilitation technique during delivery of resident care. Strives to develop a cohesive team with nursing staff. · Able to clearly communicate and direct residents in performance of restorative program. · Monitors CNA for compliance with restorative programs assigned to them. Continuous Improvement · Ensures standard precautions and infection control, isolation, fire, safety, and sanitation practices and procedures are followed by co-workers. · Participates in CrossBridge specific and organization wide Continuous Improvement (CI) initiatives. Training and Development · Attends all staff meetings and in-services designated as required or mandatory. · Attends organization wide program and meetings to be informed and current on organization strategies and programs. · Maintains competence with specific C.N.A., Occupational, and/or Physical Aide skills by performing competency training as directed by the Director of Nursing. · Assists in the teaching and directing of CNA with accurate ADL scoring, proper implementation and documentation of restorative programs in EMR. QUALIFICATIONS/REQUIREMENTS Licensure/Certification · Current State of Illinois C.N.A. certification. Education · Certificate demonstrating completion of an IDPH approved Occupational Rehabilitation and Physical Rehabilitation course. · Current CPR card. Experience · At least 1 year experience of long term care nursing as a C.N.A. · May be newly certified in Occupational Rehabilitation or Physical Rehabilitation. ENVIRONMENATL AND PHYSICAL DEMANDS: Physical Activities: Constantly standing, walking, talking, hearing, squatting, bending, squatting, reaching, twisting, driving, feeling, and grasping. Occasionally sitting, crouching, crawling, reaching hands and arms, tasting, and smelling. Range of Motion: Constantly moving from waist-shoulder height and waist and below. Assure that established infection control and universal precaution practices are maintained when performing nursing procedures. Follow established safety precautions when performing tasks and when using equipment and supplies. Assure that equipment is cleaned and prepared for the next shift. Attend nursing department, Care Plan, and other meetings as directed by the DON or ADON.
