We are looking to add to our Visiting Angels Team! PRINCIPAL ACCOUNTABILITIES : · Understand and adhere to information and precautions contained in individual client’s Care Plan. · Assist the client with personal care activities including: bathing, skin care, back rub, hair care, nail care, dressing and undressing, feeding, oral hygiene, shaving, grooming and bedtime care. · Assist the client to the bathroom or in use of urinal or bedpan. Keep incontinent clients clean and dry. · Assist the client with client's self-administration of medications. · Assist the client with personal communication skills, as needed. · Prepare appropriate documentation of the client care or service(s) furnished. · Report all client and employee incidents/accidents to supervisor immediately. · Assist the client with supervising exercise, ambulation and transfer activities. Be aware of proper body mechanics. · Ensure maintenance of a clean, safe, comfortable and healthy environment for the client. · Observe safety precautions, including: wiping up wet floors, spills and other falling hazards immediately. Also, report safety hazards, frayed electrical cords, unsecured handrails, malfunctioning smoke and carbon monoxide detectors/alarms, defective equipment or environmental hazards to supervisor on the same day of observation. · Engage in respectful social interaction with client, including friendly conversation and empathic support, as well as, respecting client’s privacy and property. · Exhibit positive attitude and behavior and demonstrate respect for employees and clients and maintain absolute confidentiality of all information pertaining to clients, clients' families and employees. · Respond and attend to client’s requests promptly. · Communicate effectively with all members of the team. · Maintain proper hand washing techniques. · Participate in required in-service training programs to meet regulatory requirements. · Understand the importance of seeking assistance, as needed, from your supervisor and/or others; demonstrate capability and dependability in following instructions. · Understand that regular, consistent attendance is necessary to serve clients. · Accept and fulfill assignments with the Agency; exercise judgment in accepting assignments. · Perform related duties and responsibilities as deemed appropriate by the management team. Listed on the state registry as a Nursing Assistant within 120 days of employment. Motivation and desire to work with geriatric persons. Ability to speak English at a level residents can understand. Ability to read and write in English to carry out functions related to the care plans and provide appropriate resident care. Physical Requirements: Must be able to constantly work on their feet during an 8 hour shift; bend, stoop, twist, and turn constantly throughout the shift in performing regular duties. Must be able to lift up to 30 pounds of resident weight and/or transfer and reposition a resident at least 25 times throughout the shift. Must be able to react quickly and decisively in emergencies and in response to the unexpected behavior of residents either during lifting or repositioning or during other frequent occasions when having direct contact with residents. Must be able to handle the physical transfer of residents from unsafe to safer locations in the event of a fire or other disaster. Ability to do sequential events/tasks. As a Home Assistance CNA you will: • Keeps Client Service Coordinators informed of schedule availability • Provides excellent personal care to clients to maximize satisfaction and follows established Plan of Care • Maintains safe environment in client’s home and provides medication reminders as instructed by the RN and listed on plan of care • Provides timely, accurate, and high quality services in accordance with client needs • Maintains AHA continuing education requirements by attending 4 mandatory staff meetings and completing the required annual in-services on-line via the learning suite modules • Consults with Clinical and office staff on client problems and is involved with long and short-term goals that are consistent with organization’s vision, mission, values and goals. • Available to run errands for clients with signed waiver • Assist clients with exercises • Turning, repositioning and transferring clients as indicated in RN’s established plan of care • General knowledge of all Activities of Daily Living (ADLs) and how to perform them. • Ability to speak, read and write English. • Ability to effectively communicate with clients and anticipate their needs. • Ability to work independently and handle a multi-task environment. • Ability to maintain confidentiality relating to client issues. • Strong customer service skills on the telephone and face-to-face contact. • Strong organizational skills. • Computer skills preferred.
