Be the Best Hospice Aide You Can Be If you meet these qualifications, we would love to meet you: Certified by the state as a nurse’s aide, with completion of required training and a passing score of 85% or higher on the agency’s competency exam; additional requirements vary by state (e.g. CHHA in CA/NJ, CGNA in MD). CPR certification required for direct patient care in Georgia and Washington. Must be listed on the state aide registry with a clear record and eligible to provide care in homes, nursing facilities, or hospitals. Requires a reliable vehicle, valid driver’s license, and state-required auto insurance. Responsibilities: Provide compassionate, hands-on care to hospice patients, including assistance with personal hygiene, repositioning, and mobility support for individuals who may require lifting (up to 200 lbs with assistance). Observe and report changes in patient condition, collaborating closely with the RN and interdisciplinary team to ensure responsive, individualized care. Support patients in their homes or care facilities through frequent movement and physical activity, such as walking, standing, reaching, and occasional driving, ensuring timely and dignified end-of-life care. Document care accurately and timely to reflect the evolving plan of care, including interventions, goals, and patient needs. Work comfortably in home and facility environments, adapting to varying conditions such as temperature changes, moderate noise, and standard healthcare exposures, while upholding infection control and safety protocols. Job Summary Summary 8 and 12 hour shifts every other weekend and holidays Grade 130 The Patient Care Assistant II functions as a member of the patient care team, under the direction of the Registered Nurse (RN), and proficiently and consistently performs all aspects of the PCA role. The Patient Care Assistant II is responsible for supporting the RN with clinical tasks, including but not limited to blood glucose monitoring, completing EKGs, applying simple dressings, using aseptic technique and responding to medical emergency situations utilizing Heartsaver CPR and AED skills. In addition, the PCA II is responsible for responding to the needs of patients and family members and taking an active role in the efficient operation of the unit. Yes Essential Functions -PROVISION OF A SAFE ENVIRONMENT • Anticipates and identifies patient safety risks, in cooperation with licensed nurse and takes appropriate action to reduce and/or eliminate the risks. For example: maintains proper bed height, uses side rails and alarms appropriately, places call light within patient reach, provides frequent patient checks and assists patient with ambulation, as indicated. • Makes routine patient room rounds to address patient needs. • Follows and maintains restraint policies, under the supervision of a licensed nurse. Recognizes and notifies RN immediately of any significant change in a patient’s condition and/or monitoring alarms (for example: change in vital signs, change in patient’s mental status, complaints of pain, abnormal blood sugar, patient fall or other injury, patient concerns with care). • Accurately collects and records patient data (vital signs, intake and output, blood sugar, weights, restraint care/monitoring, other) and complies with hospital documentation standards to ensure a complete and accurate patient record. Maintains a clean and safe working and/or living environment in the PACE center and/or participants’ homes. Assists with meal and snack preparation, serving, feeding as needed, and clean-up. Uses safe techniques in all interactions with participants at the PACE Center and in participants’ homes. Provides accurate and timely documentation in the EMR and other systems as required by SE policies and procedures and/or as assigned by supervisor. Participates in carrying out schedule of daily cleaning and disinfecting of the PACE center in accordance with CDC guidelines for increased precautions Actively participates in distribution of work for health aides to ensure care needs of participants and site tasks are completed timely and appropriately including but not limited to community-based care and center-based tasks, as assigned Actively participates in regular team meetings for health aides at respective site Participates in carrying out infection control precautions and increased cleaning and disinfecting of the PACE center as assigned and in accordance with current CDC guidelines Performs all duties in accordance with FH and Summit ElderCare policies and procedures. Qualifications Education: Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program is highly desirable, but we will accept equivalent work experience in lieu of certificate.
