Responsibilities: GENERAL PURPOSE Provides administrative leadership for all staff CRNAs. Staff CRNAs provide anesthesia services to patients of all ages at Strong Memorial Hospital under the medical direction of members of the faculty of the Department of Anesthesiology of the University of Rochester Medical Center. With the assistance of the Associate Chief(s), and Assistant Chief, and delegating specific aspects of the duties below as appropriate, the Chief CRNA will ensure the completion of the following duties and responsibilities: Leadership & Training: Orients new CRNAs to the department; completes initial competency review. In collaboration with Department Chair, leads recruitment efforts for new CRNA staff. Leades dynamic recruitment of CRNAs to maintain adequate staffing patterns, schedule structured and thoughtful interview experiences, and follow up with recruits to answer questions and provide feedback. Brings CRNA concerns to the attention of Chief Clinical Coordinator and serves as sounding board when problems arise. Coordinates annual reviews for CRNAs. Performs in-person feedback. Provides agenda items for regular meetings with Chair and Program Administrator including requests from the CRNA group. Attends and co-leads meeting. With Vice Chair for Education, selects and supervises coordinators for rotating SRNAs. Serve as Crew Chief “CC” to be a point person for the CRNAs and assist with day-to-day OR choreography. Mentorship: Serves as coach and advisor to all CRNAs and SRNAs as well as nurses interested in continuing their professional career as a future CRNA Scheduling: Develops and oversees a fair, reasonable and transparent system to schedule CRNAs to work off shifts (e.g. 3-11) and to provide coverage for residents on occasions such as graduation weekend and during residents’ exams. Oversees and assists with any scheduling changes needed to accommodate OR needs. Organizes and schedules CRNA shadows for interested ICU nurses. Works with administration to approve paperwork for non-employee shadows. Communication: Communicates updates, changes, and issues to the CRNA group. Functions as main point of contact for CRNAs and liaison to Department Leadership. Schedules regular meetings of CRNA group. Communicates with locum companies as needed to ensure staffing needs are met. Communicates regularly with all members of departmental leadership. Communicates with perioperative nursing leadership to identify areas of improvement with staff communication, physical obstructions in the OR, or surgical staffing concerns. Communicates with Human Resource Business Partner as needed to ensure compliance with hospital policies. Administrative: Completes and submits paperwork to AANA for prior approval of CEU credits for Thursday morning conferences. Handles all paperwork necessary for obtaining CEU credits. This includes collecting signatures weekly for Strong and from Sawgrass, collating the signatures, and e-mailing the totals to the AANA. In general, the chief should be responsible for this activity at one location (SMH/Sawgrass) and the Associate should be responsible at the other. Completes forms as required for CRNA and sRNA credentialing. Ensures there are supplies in the CRNA office. Diligently participates in the Executive Committee and Anesthesia & Peri-Operative Services Committee to ensure understanding of all areas and monitor changes to clinical practice. Completes Workday and UKG compensation and time approval for all CRNAs. Attends weekly and bi-weekly meetings or delegate to another participant to ensure our role is present Meets regularly with leadership (chair, vice chair and finance director) to review department finances, needs, workflow and efficiencies. Reviews annual market analysis to maintain a competitive compensation package. Ensures all CRNAs maintain compliance in annual health updates, MyPath and POCT courses and BLS/ACLS/PALS recertification. Performs other duties as assigned. Description Under the supervision of an Attending Anesthesiologist, this employee administers, monitors and manages patient’s care before and after anesthesia. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. Reports to Attending Anesthesiologists. A certain degree of creativity and latitude is required. Nurse Anesthetists will rotate responsibility for daily scheduling of case coverage. Responsibilities In conjunction with attending anesthesiologist, Nurse Anesthetist will perform a thorough and complete pre-anesthesia assessment. Nurse Anesthetist is responsible for obtaining reviewing relevant tests and discussing findings with the Attending. The Nurse Anesthetist shall obtain or verify that an informed consent has been obtained for the planned anesthetic intervention from the patient or legal guardian. Discuss anesthetic options and risks with the patient and/or legal guardian after discussion with Attending Anesthesiologist. Nurse Anesthetists must document in the patient’s medical record that informed consent was obtained. The Nurse Anesthetist, in conjunction with the Anesthesiologist, must formulate a patient-specific plan for anesthesia care. The plan is based upon comprehensive patient assessment, problem analysis, anticipated surgical or therapeutic procedure, patient and surgeon preferences and current anesthesia principles. The Nurse Anesthetist shall induce anesthesia under the supervision of an Attending Anesthesiologist and maintain anesthesia at required levels. The Nurse Anesthetist shall continuously assess the patient’s response to the anesthetic and/or surgical intervention and must immediately notify the Attending Anesthesiologist of significant changes in the patient’s condition and intervene as required, under the supervision of an Attending Anesthesiologist to maintain the patient in a satisfactory physiologic condition. The Nurse Anesthetist must monitor the patient’s physiologic condition as appropriate for the type of anesthesia and specific patient needs: A. Monitor ventilation continuously. Verify intubation of the trachea by auscultation, chest excursion, and confirmation of carbon dioxide in the expired gas. Continuously monitor end-tidal carbon dioxide during controlled or assisted ventilation. Use spirometry and ventilatory pressure monitors. B. Monitor Oxygenation continuously by clinical observation, pulse oximetry, and if indicated, arterial blood gas analysis. C. Monitor cardiovascular status continuously via electrocardiogram and heart sounds. Record blood pressure and heart rate. D. Monitor body temperature continuously on all pediatric patients receiving general anesthesia and when indicated, on all other patients. E. Monitor neuromuscular function and status when neuromuscular blocking agents are administered. F. Monitor and assess the patient positioning and protective measures. The omission of any monitoring standards shall be documented and the reason stated on the patient’s anesthesia record. The Nurse Anesthetist shall be in constant attendance of the patient until another qualified health care provider has accepted the responsibility for care. 6. The Nurse Anesthetist is responsible for maintaining complete, accurate, and timely documentation of pertinent information on the patient’s medical record. Must document all anesthetic interventions, and patient responses. 7. The Nurse Anesthetist is responsible for transferring the responsibility for care of the patient to other qualified providers in a manner that assures continuity of care and patient safety. 8. The Nurse Anesthetist must adhere to appropriate safety precautions, as established within the institution, to minimize the risks of fire, explosion, electrical shock and equipment malfunction. Document on the patient’s medical record that the anesthesia machine and equipment were checked. Prior to use, the Nurse Anesthetist shall inspect the anesthesia machine and monitors according to established guidelines. The Nurse Anesthetist shall check readiness, availability, cleanliness, and working condition of all equipment to be utilized in the administration of the anesthesia care. When the patient is ventilated by an automatic mechanical ventilator, monitor the integrity of the breathing system with a device capable of detecting a disconnection by emitting an audible alarm. Monitor oxygen concentration continuously with an oxygen supply failure alarm system. 9. Precautions shall be taken to minimize the risk of infection to the patient, the Nurse Anesthetist, and other healthcare providers. 10. The Nurse Anesthetist shall participate in the ongoing review and evaluation of the quality and appropriateness of the anesthesia care as a member of the Department of Anesthesiology. Evaluation shall be performed based upon appropriate outcome criteria and reviewed on an ongoing basis. The Nurse Anesthetist shall participate in a continual process of self-evaluation and strive to incorporate new techniques and knowledge into the practice. 11. The Nurse Anesthetist will participate in departmental Grand Rounds, presenting their cases as appropriate. 12. The Nurse Anesthetist shall respect and maintain the basic rights of patients. 13. The Nurse Anesthetist assists in research projects as directed by an anesthesiologist. 14. Principal Duties and Responsibilities Clinical Practice 1. In conjunction with Attending Anesthesiologist, performs a thorough and completed pre-anesthesia assessment. Responsible for obtaining and reviewing relevant tests and discussing findings with the Attending. 2. Obtains and/or verifies that an informed consent has been obtained for the planned anesthetic intervention from the patient or legal guardian. Discusses anesthetic options and risks with the patient and/or legal guardian after discussion with Attending Anesthesiologist. Documents in the patients' medical record that informed consent was obtained. 3. Formulates a patient-specific plan for anesthesia care in conjunction with the Anesthesiologist. The plan is based upon comprehensive patient assessment, problem analysis, anticipated surgical or therapeutic procedure, patient and surgeon preferences, and current anesthesia principles. 4. Induces anesthesia under the supervision of an Attending Anesthesiologist and maintains anesthesia at required levels. Assesses continuously the patient's response to the anesthetic and/or surgical intervention and must immediately notify the Attending Anesthesiologist of significant changes in the patient's condition and intervenes as required, under the supervision of an Attending Anesthesiologist to maintain the patient in a satisfactory physiologic condition. 5. Monitors the patient's physiologic condition as appropriate for the type of anesthesia and specific patient needs. * Monitors ventilation continuously. Verifies intubation of the trachea by auscultation, chest excursion, and confirmation of carbon dioxide in the expired gas. Continuously monitors end-tidal carbon dioxide during controlled or assisted ventilation. Uses spirometry and ventilatory pressure monitors. * Monitors oxygenation continuously by clinical observation, pulse oximetry, and if indicated, arterial blood gas analysis. * Monitors cardiovascular status continuously via electrocardiogram and when appropriate, heart sounds. * Records blood pressure and heart rate. * Monitors body temperature continuously on all pediatric patients receiving general anesthesia and when indicated, on all other patients. * Monitors neuromuscular function and status when neuromuscular blocking agents are administered. * Monitors and assesses the patient positioning and protective measures. The omission of any monitoring standards shall be documented and the reason stated on the patients anesthesia record. The CRNA shall be in constant attendance of the patient until another qualified health care provider has accepted the responsibility for care. 6. Maintains complete, accurate, and timely documentation of pertinent information on the patients' medical record. Must document all anesthetic interventions and patient responses. 7. Transfers the responsibility for care of the patient to other qualified providers in a manner that assures continuity of care and patient safety. 8. Adheres to appropriate safety precautions, as established within the institution, to minimize the risk of fire, explosion, electrical shock and equipment malfunction. Responsibilities: - Selects and implements appropriate anesthetic agent. - Initiates and maintains patient ventilation when appropriate. - Provides for protection of the patient's eyes and pressure points when appropriate. - Observes and records the effects of ambient temperature on the patient when appropriate. - Evaluates the adequacy of anesthetic depth. - Evaluates, records, and treats any cardiovascular dysrhythmia. - Evaluates, manages, and documents fluid and electrolyte status. - Identifies and determines the need for blood component therapy. - Effectively adjusts the anesthetic to meet requirements of patient need. - Evaluates the need for drug antagonism and reversal. - Recognizes and reports any abnormal patient responses.
