Coordinate and communicate resident care needs with physicians, families, pharmacies, and other care providers. Support health promotion, disease prevention, and wellness initiatives within the community. Ensure accurate, timely, and compliant clinical documentation. Assist with care planning, service plan updates, and resident evaluations as required. Collaborate with caregivers, wellness team members, and leadership to ensure consistent, high-quality care delivery. Respond to resident concerns, incidents, and emergencies using sound clinical judgment. Support training and guidance for care staff as appropriate. Ensure the community adheres to applicable federal, state, and local regulations related to resident care and services. Participate in quality assurance initiatives, audits, and compliance activities as needed. Perform other duties as assigned. Qualifications: Current Licensed Practical Nurse (LPN) license, or a Professional Registered Nurse as per state regulations Minimum of two (2) years of Licensed Nurse experience required. Experience in assisted living, long-term care, or a skilled nursing environment preferred. Strong clinical judgment, problem-solving, and decision-making skills. Effective organizational, time management, and communication skills. Ability to manage multiple priorities and delegate appropriately. Demonstrated commitment to seniors and person-centered care. Ability to work nights, weekends, and flexible schedules as required by the community. Proficiency with electronic documentation systems and basic computer applications. Ability to respond to medical emergencies. Exposure to bloodborne pathogens and infectious disease. Ability to lift or assist residents as required by care plans.
