MDS COORDINATOR- Exempt (Salary) Position Reports to Administrator OVERVIEW: MDS Coordinator administers patient assessments and overseas the assessment process, setting the assessment schedules and assuring that assessments are done in an accurate and timely manner. The MDS Coordinator coordinates the care plan according to regulatory requirements. Ensures that resources are made available to patients and patient care is delivered effectively and to a satisfactory standard. Creates the schedule for all Medicare and Medicaid. Start Medicare coverage for newly qualified patients and remain updated on changes in Medicare coverage and help determine documents needed for reimbursement. RESPONSIBILITIES: Oversees accurate and through completeion of the MDS, Care Area Assesments (CAA) and Care Plans in accordance with federal and state regulations and guidlines that govern the process. Acts as an in-house Case Manager demonstrating detailed knowledge of residents' health status, critical thinking skills to develop an appropriate care pathway and timely communication of needed information to the resident, family, other health care professionals and third- party payers. Proactively communicates with Administrator and Dircetor of Nursing to identify regulatory risk that allow capture of resources provided on the MDS, and clinical trends that impact resident care. Demonstrates an understanding of MDS requirements related to varied payers including Medicare, Medicaid, and Managed Care. Ensures timely electronic submission of all MDS to the state database. Reviews state validation reports and ensures that the appropriate follow up action is taken. Facilities the Care Management Process engaging the resident, IDT, and family in timely identification and resolution of barriers to discharge resulting in optimal resident outcomes and safe transition to the next care setting. Directly educates or provides company resources to the IDT members to ensure thay are knowledgeable of the RAI process. Analyzes QI/QM data in conjunction with the IDT members to identify trends. QUALIFICATIONS: Proficiency in MDS 3.0. Demonstrating knowledge of state and federal regulations. Registered Nurse or LPN with current, active license in the state. Minimum two years of clinical experience in LTC setting. PHYSICAL REQUIREMENTS: This position is very active and requires standing for long periods of time, walking throughout facility, bending, kneeling, stooping all day. Clerical and Administrative Duties • Responsible for clinical and clerical functions of the respective unit. • Performs secretarial duties for the unit including but not limited to faxing, copying, mailing, and typing any essential material. • Answers telephone promptly and courteously using proper telephone etiquette and routes calls and messages appropriately. • Ability to work both in the clinical and clerical setting. • Maintains chart organization per established procedures. • Maintains the orderliness of medical records in the electronic health record by ensuring accuracy of any outside records that are uploaded to the system. • Requests results, labs, and other patient-related records from outside offices. • Follows-up with patients to ensure that pre-op testing is done in a timely manner according to the scheduled date of surgery. • Contacts surgical patients for their scheduled arrival time and location the day before surgery. • Adheres to HIPPA standards. • Follows-up with medical doctor’s offices to ensure that all of the pre-op results are sent to the UPAC department in a timely manner to ensure that the patent can proceed with the scheduled surgical procedure. • Communicates closely with the RNs and APPs to ensure that the correct results/records have been obtained, reviewed, and finalized in a timely manner. • Reviews clinical records for completeness and to identify missing data (labs, EKGs, X-Rays, Cardiac Testing, Medical Clearances, etc.) in a timely manner to ensure that the patient can proceed with the scheduled surgical procedure. • Communicates with RNs, APPs, and Manager in a timely manner regarding missing reports. • Communicates concerns and changes to RNs, APPs, and Manager. • Delivers medical reports to RNs and APPs for review. • Accesses the computer system to obtain medical records, including UChart, Lab Corp, Quest etc. • Maintains effective communication with patients, physicians, RNs, APPs, and staff members. • Implements appropriate preventative measure for patients identified with safety risks (i.e. falls). • Schedules follow-up appointments and consults ensuring patient has appropriate information and provide as needed. • Assists with scheduling as needed for labs, or clinic appointments as needed. • Retrieves and/or assists clinical personnel in retrieving diagnostic studies as needed. • Retrieves and prints schedules for the next day. • Orders supplies for supply room as needed. • Maintains area stocked with the necessary supplies and forms. Professional Accountability • Consistently maintains complete confidentiality of all medical, financial, computer, and other sensitive materials to maintain patient privacy. • Acts as a role model and always maintains a professional demeanor. • Develops and maintains clinical competency. • Maintains professional certifications and licensure in active status. • Maintains documentation of continuing education requirements. • Attends all mandatory in-services and departmental meetings. • Actively supports and participates in performance improvement activities. • Seeks/accepts training to learn new skills and maintain current competencies. • Practices within the guidelines and scope of a certified nursing assistant. • Displays courtesy toward customers by introducing self and addressing customer by name. • Meets customers’ expectations by listening to them, demonstrating sensitivity, and awareness of others. • Follows through to ensure that established expectations are satisfied; keep customers informed of progress. • Works effectively and cooperatively in groups/teams to ensure quality patient care. • Communicates effectively with the manager and other members of the interdisciplinary team. • Attempts to resolve patient, physician, and visitor complaints and refers unresolved issues to managers. • Demonstrates dependability and reliability by promptly reporting to work when scheduled. • Serves as a mentor and role model. Computer Skills: Demonstrates proficiency in PC applications.
