The Home Care Services Program (HCSP) is a Medicaid funded program that provides long term care to medically fragile and vulnerable individuals who require assistance with activities of daily living through the Community Alternative Systems Agency (CASA), Managed Long Term Care (MLTC) Program, the Assisted Living Program, Care at Home Program and Homebound Medicaid. The Medicaid Eligibility Unit is responsible for the Medicaid eligibility review and processing of initial applications, change actions, and renewals for all Medicaid funded long term care programs. This unit is responsible for the processing of Medicaid eligibility for consumers who receive services through one of the HCSP programs. Home Care Services Program (HCSP) is recruiting two (2) Staff Nurses, to function as Staff Nurses in HCSP/ CASA 11 – Care at Home/ Home Bound Unit, who will: - Conduct in-home or virtual assessments at the request of HRA using a State approved assessment instrument to evaluate the client’s ability to safely perform activities of daily living and the degree to which the client requires assistance for each function and task. The completed assessment would be entered into the New York State Department of Health (NYSDOH) and/or HRA computer system. - Assist in determining the level of home care services or other services of care that the client needs based on the medical diagnosis and degree of functional impairment. The development of a plan of care that takes into consideration client’s home environment and social support system. - Complete in-home and virtual visits and submit the completed HRA/NYSDOH approved assessments and recommendations forms no later than five (5) workdays after the date of assignment. - Conduct case reviews of new applicants, service change and re-authorization cases, and participate in special projects. The reviews consist of screening all documents related to the request for service and may include contacting the client’s physician or other resources for additional medical documentation and information as necessary. - Develop a plan of care for the type and amount of home care services to be provided to the client after reviewing all required case documentation. - Prepare documentation for and testify at Fair Hearings and at other legal proceedings to explain clinical findings and determinations regarding the plan of care. - Provide consultation to HRA Medical Review Team and social service staff at HRA’s request. - Complete self-audits of State approved assessments and medical review of charts. - Work toward obtaining PRI/screen certification and conduct assessments of clients transitioning to a Nursing Home Facility. The Prep & Recovery Unit RN cares for patients who will be receiving anesthetic care and who are recovering from anesthesia after surgery. They monitor pre and post-operation vital signs, assess levels of consciousness, and carefully observe patients for any side effects from anesthesia. Roles and Responsibilities: Performs and documents an accurate assessment on each assigned patient within established standard of care. Identifies a patient's immediate needs on admission and establishes daily goals. Assessment of any allergies and/or infection control techniques. Knows what procedures will be and has been performed. Able to recognize priorities within patient care and act accordingly especially with pain management and documentation. Completes Phase I and/or II record at end of each case. Demonstrates accuracy in data collection with utilization of Ambulatory Surgery record and patient chart for: H&P and Lab Values Confirms surgical outcome with anesthesia and any event with anesthesiologist post op. Documents flow sheets, graphic sheets, and anesthesia signature are completed by end of recovery phase and reflects patients progress toward established goals. Ability to appropriately problem solve incidents on the unit. Communicates incident information and action taken to the appropriate people. 4 - 10 or 3 -13 hour variable shifts. Unit operating our hours 5a-10p. Shifts will flex between pre-op and post-op and will be based on surgical schedule. Call component is included in this role. One weekend 24 hour call shift in a 6 week rotation. Weekday call begins at 9p-7am. Some holiday call.
