Nurse Practitioner / Physician Assistant | Value Based Care - Bridge Clinic | Day | Full-Time

Overview This position is fully in the ambulatory or outpatient setting, with no inpatient responsibilities. The clinician will play an essential role on a multidisciplinary team, assisting in the delivery of care for patients undergoing care transitions (e.g., from the emergency department or inpatient stay to home), managing follow-up for chronic diseases, and closing health maintenance gaps. The role involves providing comprehensive diagnostic and therapeutic health care services to patients, including but not limited to: taking medical histories, performing medication reconciliation, conducting physical examinations, ordering and coordinating lab tests and diagnostic studies, prescribing medications, and guiding patients on treatment plans and health education. Specific responsibilities include conducting post-discharge or transitional care management visits, Medicare Annual Wellness Visits, and chronic disease management visits. This work is conducted under the direction of UFHP and ambulatory care leadership and will interface with interdisciplinary teams across the health system. Patient interactions will primarily occur through telemedicine visits and telephone outreach (with the potential to see patients in-person one day per week), requiring strong electronic medical record (EMR) proficiency and excellent telemedicine communication skills with both patients and team members. Qualifications Minimum Education and Experience Requirements: Master's degree in Nursing or Physician Assistant. Credentialing: Must meet requirements set forth by the Medical Staff Office to obtain and retain privileges to practice as an Advanced Practice Registered Nurse or Physician Assistant within UF Health Shands Hospital and/or the Shands UF Healthcare Network, as applicable. Day Center CNA

The Day Center CNA ( Certified Nurse Assistant) works at our Adult Day Health Center assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care. Follow the care plan and assist participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting. Document care provided in accordance with established policies, guidelines, and applicable regulations. Carry out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews. Assist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed. Monitor participants health status such as vital signs, weights, skin condition, appetite and changes in functional ability and report pertinent information to clinical supervisor in a timely manner. Collaborate with multidisciplinary teams in care planning for participants. Monitor appetite and look for changes in functional ability and skin condition. Monitor/assist participants in a courteous manner with meals and helps with feeding, as necessary. Report any variance, accident, or unusual occurrence to the appropriate personnel. Assist with loading and unloading of participants from vehicles. Assist the transportation department with participants’ take-home supplies. Serve as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments supporting participants while in transit and accompanying participants in and out of facilities. Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar when needed. Attend in-service and continuing education programs as scheduled and necessary. Other duties as assigned. REQUIRED Active and valid state-issued Certified Nursing Assistant (CNA) license Current CPR certification or the ability to obtain CPR certification. Acceptable vendors for certifications are from either American Heart Association and/or American Red Cross.
