Assist residents in ambulation with or without self-help devices as instructed Performs restorative procedures as instructed Accurately measure and record heights and weights, temperatures, pulses, respirations, and blood pressures as instructed Empty urinary drainage bags and record amount in the record as indicated; report any irregularities to nurse Ensure call lights are within reach of residents and answer call lights promptly Ensure that residents who are unable to call for help are checked frequently Report all changes in residents’ condition to supervisor as soon as practical Maintain cleanliness of resident units, central bathing area, treatment and supply rooms, workrooms, and linen closets; clean bedpans, urinals, and commodes; wash basins, sanitize bathtub after each resident’s use, clean floors Perform other related duties as assigned. Summary The Oncology Nurse Navigator 2 is a registered nurse (RN) who works with physician practices and multi-disciplinary teams to promote the effective utilization of services and coordination of care for patients within the Cancer Center service line. The Oncology Nurse Navigator 2 contributes to the assessment of patients' psychosocial, financial and clinical needs throughout the continuum of care ensuring high quality care while supporting quality metrics of the Cancer Center and Rush University Medical Center The Oncology Nurse Navigator 2 functions as a liaison between patients, physician practices, the hospital, and the community performing functions that meet the core competencies of oncology navigation; Care Coordination, Communication, Education, and Professional Role/Care as well as the domains of the Profession Practice Model; Relationships and Caring, Evidence Based Practice, Technical Expertise, Critical Thinking and Leadership. Our goal is to reduce oncology readmissions and for this clinic to be the bridge between inpatient and outpatient. The goal is to build relationships with our Cancer Center patients and their loved ones. This nurse will also build relationships with case managers, nurses, providers and leadership on the units to encourage referrals to the clinic.
