Position Responsibilities As the MDS Coordinator, you will: Coordinate the MDS assessment process for residents throughout their stay. Ensure accurate and timely completion of required MDS assessments, including Medicare/PPS, quarterly, annual, and significant change assessments. Coordinate interdisciplinary team participation in the MDS process. Maintain an accurate MDS calendar and assessment schedule, including appropriate Assessment Reference Dates (ARDs). Review documentation to support accurate MDS coding and reimbursement. Complete MDS data entry and electronic submission. Verify successful electronic submission of assessments. Complete corrections and modifications when necessary. Maintain accurate and organized MDS records and supporting documentation. Collaborate with nursing, therapy, dietary, social services, activities, and other interdisciplinary team members. Monitor assessment completion to ensure regulatory and facility deadlines are met. Assist with identifying changes in resident condition that may require an MDS assessment. Maintain knowledge of current CMS and regulatory requirements related to the MDS process. Participate in care planning and interdisciplinary team meetings as appropriate. Promote accurate documentation and quality outcomes for our residents. Qualifications Required: Current, active, and unencumbered Illinois LPN or RN license Strong organizational and time-management skills Ability to communicate effectively with residents, families, and interdisciplinary team members Ability to read, write, speak, and understand English Strong attention to detail and accuracy Ability to work independently while also functioning effectively as part of a team Preferred: Previous MDS experience Long-term care/skilled nursing experience Knowledge of CMS regulations and the RAI/MDS process Experience with electronic MDS submission systems We are willing to consider an experienced LPN or RN who is looking to grow into the MDS Coordinator role.
