Job Description Distinguishing Characteristics All positions are in the non-competitive jurisdictional class. Nursing Assistant 1: entry level; performs basic patient care duties under direction of a nurse, physician, or other clinician; may assist Nursing Assistants 2, physicians, and nurses in the performance of care functions. Incumbents at this level receive in-service training in a limited number of care procedures, leading to eligibility to qualify for appointment to Nursing Assistant 2. Nursing Assistant 2: full performance level; using detailed guidelines, standards, and procedures, and under the supervision of clinicians, observes patients and performs prescribed simple treatment procedures; attends to personal hygiene needs of patients; and performs limited cleaning and housekeeping duties directly related to patient care. Treatment tasks performed require only knowledge of a limited number of specific treatment procedures performed at the facility. Nursing Assistant (Certified): full performance level; performs duties similar to a Nursing Assistant 2, but functions are performed in a long-term care facility and require certification as a New York State Certified Nursing Assistant. Related Classes Rehabilitation Hospital Nursing Assistants perform similar functions to those of Nursing Assistants but are focused on rehabilitation activities and are only classified at DOH’s Helen Hayes Hospital. Licensed Practical Nurses are classified in a variety of State health care settings and observe patients for symptoms and reactions to treatments; treat patients for physical maladies as directed and under close supervision; and attend to patients’ personal hygiene needs. Incumbents are licensed New York State Licensed Practical Nurses. Illustrative Duties Nursing Assistant 1 Under Direct Clinical Supervision, Performs The Following Duties Changes simple dressings as directed, gives enemas, douches, and irrigations as directed. Applies abdominal “T” binders, hot water bottles, aqua “K” pads, ice bags, and coolers, and hot and cold compresses to patients as directed. Assists patients in collecting urine and stool specimens. Observes and records patients’ quantitative food and fluid intake and waste output. Takes temperature, pulse rate, blood pressure and respiration rate. Gives therapeutic baths such as sitz and fever sponge baths. Measures and records height and weight. Inspects functioning oxygen therapy equipment including tents, masks, and cannulae to ensure appropriate functioning. Recognizes and reports abnormal signs and symptoms of common diseases and conditions to clinical staff. Applies splints, braces, slings and other self-help devices. Operates a variety of specialized patient monitoring and treatment devices. Attends to patients’ hygiene and daily living needs in order to maintain an atmosphere conducive to patient comfort and treatment. Examples include bathing and dressing patients, brushing teeth, and trimming nails. Collects and carries patients’ laundry and dry cleaning and delivers objects to appropriate locations in the health care facility. Receives patients’ belongings and transports objects to safe storage, keeping appropriate records. Feeds patients requiring assistance. As directed by clinical staff, moves, positions, lifts and transports patients using a variety of lifts and other aids in order to make patients comfortable, prepare patients for treatment, deliver patients to treatment areas, and to prevent contractures. Brings bedpans and urinals to patients requiring assistance and attends to incontinent patients. Talks and listens to patients and responds by encouraging patients to cooperate and participate in treatment activities. May transport patients and supplies on occasion. May perform a variety of limited housekeeping and cleaning duties related to direct patient care in order to ensure that treatment and patient areas are clean, sanitary, orderly, and properly supplied. Nursing Assistant 2 Under direction of higher-level treatment staff, performs duties similar to those of a Nursing Assistant 1, but with greater proficiency. Positions may assist with training new staff in assigned treatment areas. Nursing Assistant (Certified) Under direction of higher-level treatment staff, performs duties similar to those of a Nursing Assistant 2, but also participates in required quarterly in-service education to maintain certification and meet all necessary requirements to be re-certified every two years. Communication Nursing Assistants have frequent face-to-face communication with patients and visitors for the purposes of conveying treatment procedure and visitor information. Incumbents attempt to encourage and support patients who may be despondent or apprehensive, and regularly communicate with clinical staff to convey information on patients’ physical conditions and responses to treatment. This information includes items such as observable physical behavioral changes, fatigue levels, and general behaviors and responses to routine treatments, such as foot soaks. Qualifications Nursing Assistant 1 Non-Competitive: possession of or ability to acquire the necessary knowledge, skills and abilities during the probationary period. Nursing Assistant 2 Non-Competitive: one year of satisfactory full-time experience in the provision of simple treatment and other patient care services under professional supervision. Possession of current certification as a New York State Certified Nursing Assistant may substitute for the required experience. Meal Preparation and Hydration Depending on the assignment, caregivers may be responsible for: Planning meals according to client preferences Following approved dietary instructions Preparing meals and snacks Assisting with feeding Encouraging regular fluid intake Observing and documenting food and fluid intake Assisting with grocery organization Following safe food-handling practices Washing dishes Cleaning meal-preparation areas Reporting concerns related to appetite, swallowing or hydration Difficulty swallowing, coughing during meals, choking, vomiting or significant refusal to eat or drink must be reported immediately. Light Housekeeping Caregivers may be responsible for maintaining a clean, safe and organized client environment. Responsibilities may include: Tidying the client’s immediate living space Dusting Sweeping Vacuuming Mopping approved areas Washing dishes Cleaning frequently touched surfaces Laundry and linen changes Cleaning the client’s bathroom Organizing care supplies Removing trash Keeping walkways and emergency-access areas clear Reporting unsafe conditions or needed repairs Housekeeping responsibilities are limited to the client’s approved service area and assigned care-plan tasks. Non-Medical Health Support Helping Hearts provides non-medical personal support. Caregivers must remain within their authorized role and follow the client’s approved care plan. Depending on the assignment, responsibilities may include: Providing medication reminders Assisting with approved blood glucose monitoring Documenting readings when required Observing food and fluid intake Monitoring the client’s physical, emotional and behavioral condition Reporting pain, swelling, bruising, redness or skin concerns Reporting falls, injuries or changes in mobility Reporting confusion or unusual behavior Following established emergency procedures Contacting the Helping Hearts office immediately when concerns arise Completing accurate electronic documentation Caregivers must report concerns rather than independently diagnose or treat them. Caregivers must not assume that someone else has already reported a change in condition. Medication Reminders Medication support is limited to the responsibilities authorized in the client’s approved non-medical care plan. Caregivers may: Provide scheduled reminders. Assist with an approved prefilled medication planner. Place approved medications in a cup when directed. Remind the client to self-administer their medication. Document reminders and client responses. Report refusals, missed doses or other concerns. Caregivers must not independently administer medication, change medication instructions or provide medical advice outside their authorized scope. Transportation When required and authorized, caregivers may: Transport clients to appointments. Assist with approved errands. Transport clients to social activities. Help clients enter and exit the vehicle. Secure mobility equipment appropriately. Follow all client-specific transportation instructions. Maintain a clean, safe and insured vehicle. Report any transportation concern immediately. Caregivers may only transport clients when the assignment has been approved and all transportation requirements have been satisfied. Night and Overnight Shift Expectations Caregivers working night or overnight assignments must remain fully awake, alert and attentive throughout the entire shift. Notify the office if a client or family asks them to arrive late, leave early or change the scheduled hours. Complete all required documentation before the end of the shift. Failure to report for an accepted shift, leaving a client without authorization or failing to follow safety procedures may result in removal from the assignment or additional corrective action. Assists the Executive Director/Administrator in the preparation and administration of the organization's budget. 6. Interprets operational indicators to detect census changes and increases or decreases in volume that could impact staffing levels, revenues or expenses. 7. Evaluates performance of Clinical Supervisors. 8. Assists Clinical Supervisors to develop skills and techniques in evaluating the performance of clinicians. 9. Hires, evaluates, and terminates organization personnel. 10. Conducts annual evaluations on clinicians, or more frequently if indicated. 11. Oversees the maintenance of patient clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities. 12. Assures proper maintenance of clinical records in compliance with local, state and federal laws 13. Responsible for the maintenance of adequate and appropriate inventory supplies and equipment for the provision of patient services. 14. Develops, implements and evaluates the orientation program for new organization personnel. Responsible for orientation of new organization personnel, directly or delegated. 15. Plans and implements in-service and continuing education programs to meet education and training needs of organization personnel at least quarterly. 16. Assists with the evaluation of organization performance via performance improvement program, productivity, quarterly and annual reviews. Assures for the quality and safe delivery of hospice services provided through the Organization. 17. Assists in the development of organization goals. Develops, recommends, and administers Organization policies and procedures. 18. Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel and, maintains compliance to the CHAP Home Care standards. 19. Stays informed about changes in the field of nursing and hospice care; shares information with appropriate organization personnel. 20. Promotes hospice referrals in the health care community. 21. In the absence of the Executive Director/Administrator, the Clinical Director/Director of Patient Care Services will become the acting Executive Director/Administrator and will be vested with authority to act in behalf of the Executive Director/Administrator. 22. In the temporary absence (in excess of twenty [20] consecutive working days) of the Clinical Director/Director of Patient Care Services, a Registered Nurse shall be designated in writing as responsible for fulfilling the duties of Clinical Director/Director of Patient Care Services. 23. Any vacancy in the Clinical Director/Director of Patient Care Services position shall be filled within sixty (60) days of the vacancy and the Department notified of the replacement. 24. Other duties as assigned by the Executive Director/Administrator. 25. Responsible for the coordination and integration of nursing services with other hospice services to provide a continuum of care for the patient. 26. Responsible for the development of protocols for regular verbal communication, including case conferencing, between the nursing service and other disciplines based on the needs of each patient. 27. Responsible for the development of written job descriptions and performance criteria for nursing personnel, and assigning duties based upon education,training, competencies and job descriptions. 28. Ensures that nursing services are provided to the patient as specified in the nursing plan of care. POSITION QUALIFICATIONS 1. Registered nurse in the State with current licensure to practice professional nursing in the State. 2. Bachelor’s degree in Nursing from an accredited program by the National League for Nursing. Master’s degree in hospice preferred. 3. Three years of management experience within the last five (5) years in a hospice or related health care organization, at least one (1) year of which was in a supervisory or administrative capacity, OR A Registered Nurse with four (4) years’ experience within the last five (5) years in a hospice, home health agency, primary care clinic or health facility, at least one (1) year of which was in a supervisory or administrative capacity. 4. Demonstrated ability to supervise and direct professional and administrative personnel. 5. Ability to market and deal tactfully with customers and the community. 6. Must be a licensed driver with an automobile that is insured in accordance with state and/or Organization requirements and is in good working order. 7. Has excellent observation, verbal and written communication skills. 8. Knowledge of business management, governmental regulations and CHAP standards.
