The RN Case Manager provides ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs. The Registered Nurse Case Manager enhances the quality of patient management and satisfaction, to promote continuity of care and cost-effectiveness through the integration of functions of case management, utilization review, and management, and discharge planning. Requirements Education: Clinical - Degree in Nursing, BSN preferred. Non-Clinical - Must possess one of the following MSW or LCSW. Licenses/Certification: Current license as a Registered Nurse - RN, LPN or LCSW in the District of Columbia. Experience: Minimum of 3 years of clinical experience. Case Management experience in LTAC preferred.
