The Case Manager provides evidence-based, patient-centered care at the patient’s place of residence, using critical thinking and decision-making skills. Qualifications REQUIRED QUALIFICATIONS: Education: Completion of nursing program Experience: Experience as RN in a healthcare environment for two years or more. Valid State Driver's License with proof of active auto insurance. Other Skills/ Knowledge: Excellent interpersonal and communication skills.
