ESSENTIAL JOB FUNCTIONS To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below: Provides basic care and medical treatment to individuals receiving services. Completes health assessments and provides input of health diagnosis to RN or physician for the development of nursing care plan. Follows-up on all physician orders and monitors and ensures follow through on individual’s medical/dental appointments. Monitors and records medications in a timely manner; acts as a liaison to pharmacy. Reviews medication error forms, analyzes errors and makes recommendations for procedural changes. Administers medication as required. Oversees and monitors staffs who administer medication. Conducts quality assurance checks on health care records. Provides emergency coverage and/or care for individuals receiving services. Maintains confidentiality, respects human rights and practices universal precautions in accordance with Mentor policy. Upholds policy and procedure development related to health care. Works with external agencies to coordinate services and ensure consistent treatment of clients. Administers, tracks, and maintains log for staff and individual’s testing. Provides training for staff including medication administration, universal precautions and infection control. Provides training for individuals receiving services such as self medication, prescribed treatments, sex education/STD prevention and smoking cessation. May be required to coordinate, organize and/or assist with household activities such as light housekeeping and meal preparation; transport individuals receiving services to planned and/or necessary activities, appointments. Participates in on call system as needed. Performs other related duties and activities as required. RN Supervisor Responsibilities: Collaborate with the Patient Coordinator and Clinical Manager to ensure accurate clinical documentation for reassessments and admissions. Initiate and revise the plan of care, medical orders, Medication Administration Records (MAR), and Treatment Administration Records (TAR) as needed. Conduct monthly client visits both in-home and telephonic as required. Provide in-home supervision every 60 days or as necessary. Participate in formal and informal problem-solving processes to address EMR computer education needs and achieve established outcomes for users. Act as a liaison between Summit and the community, clients, facilities, etc. Evaluate the outcome of care and provide support to ensure that services meet the client’s needs. Inform Managers and other staff of changes in the client’s needs. RN Supervisor Requirements: Current RN license in the state. Willing to travel up to an hour in distance. Job Summary: Evaluate and admit patients to home care; develop plan of care and provide care to patient according to plan. Act as case manager making necessary adjustments in the patient's plan of care related to symptom changes and changes in care needs. Review and plan patient care and or discharge during scheduled case conference meeting and document accordingly. Act as liaison for patient/caregiver with MD and community healthcare partners. Strive to lower cost of care through managing mileage, supplies and visit scheduling. Completes all documentation related to patient contact, assessments, SOC, and recerts. Resumption of care and discharge according to policy. Maintain understanding of current home care regulatory guidelines. Provide on-call coverage for both home care and hospice as assigned. Education: Associate's degree in nursing and currently licensed by the state of Alabama required.
