Required Qualifications • At least 2 years experience in health care, with at least 1 year of experience in hospital discharge planning, care management or behavioral health setting, or equivalent combination of relevant education and experience. • Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN). The nurse assists clients in health maintenance and disease prevention to improve their ability to live independently at home. The nurse directs the efforts of the Caregiver when involved in the client’s plan of care. Based on state regulations, may perform administrative and/or field/clinical responsibilities with clinical direction from a Registered Nurse.
