Location: Acute Care Hospital Tower Shift- 12hr Days (7am-7pm) Work Hours: This program is a 7 day a week service, rotating every other weekend and rotating holidays Employment Type: Full Time Hospital at Home is an innovative program that combines in-home with remote monitoring, offering patients timely, hospital-level care in their homes. Though the list of diagnoses enrolled in Hospital at Home continues to expand, common disease states include acute pneumonia, cellulitis, COVID-19, urinary tract infection, dehydration, or exacerbations of chronic conditions, such as heart failure or chronic obstructive pulmonary disease. For many patients, the program provides an alternative to the traditional hospital setting and results in improved patient outcomes and satisfaction. The Hospital at Home team is looking for motivated nurses with critical thinking skills, acute care experience, and an interest in serving the Parkland community. Primary Purpose The RN is a licensed professional who uses the Parkland nursing professional practice model to coordinate acute patient care delivery in the patient home. Using the nursing process, the RN assesses the patient in their environment, identifies nursing diagnoses based on responses to health problems, develops and implements an individualized plan of care, and evaluates the patient's response. The RN utilizes knowledge of patient needs and providing healthcare in the home environment to assist patients to transition through the healthcare encounter without any preventable complications or delays. The RN delegates interventions to health care personnel based on the Texas Nursing Practice Act, each patient's condition, and the competencies of the employee. Minimum Specifications Education Must be a graduate from an accredited school of nursing. Prefer Bachelor of Science in Nursing. Essential Functions & Responsibilities Clinical Care & Care Coordination Perform comprehensive assessments, reassessments, and ongoing monitoring to identify needs, changes in condition, and appropriate level of care; update Plans of Care accordingly. Implement provider orders (MD/DO/NP/PA), including administering medications, treatments, wound care, and other skilled nursing interventions. Triage participant and staff calls to determine priority and route/escalate appropriately. Provide medication administration, reconciliation, and training in self-administration for non-scheduled medications; monitor and promote adherence. Educate participants and caregivers/families on conditions, medications, equipment, safety, and disease management. Schedule and coordinate outside specialist visits, diagnostics, and hospital admissions as needed; arrange transportation when applicable. Observe, record, and promptly report participant condition and response to treatments/medications to providers.
