Strong communication and organizational skills are essential, along with proficiency in Microsoft Office Suite and EMR systems. The ability to collaborate effectively within an interdisciplinary team is required. This role involves regular physical activity, including lifting patients over 200 lbs with assistance, transporting supplies and equipment up to 20 lbs for distances of 500 feet, and frequent standing, walking, sitting, stooping, and reaching. Responsibilities: Conducts comprehensive nursing assessments and collaborates with patients, families, physicians, and the Interdisciplinary Group (IDG) to develop, implement, and evaluate individualized plans of care in alignment with regulatory requirements and the Case Management Recipe. Maintains timely and accurate documentation, ensures care plans reflect current patient status, and communicates effectively with the care team to coordinate optimal use of resources, especially in complex or high-risk situations. Provides education and emotional support to patients and caregivers based on individual needs, and actively participates in on-call rotations, admissions, and care transitions as directed. Supervises on-call Nurses: Monitors/evaluates performance, provides guidance, recommends hiring and termination, as needed. Determines/Deploys on-call Nurse for home visits, provides necessary information and follows-up regarding status of visit and to obtain necessary report. Provides verbal report on significant events, plan of care changes and other patient/caregiver issues requiring further action and/or follow-up to each respective team. Participates in Interdisciplinary Hospice Team meetings, management meets and serves on relevant committees for the purposes of information exchange, team collaboration, development of procedures and documentation tools, development of quality and training. Understands/implements/complies with Hospice Care Mission, objectives, policies, and procedures to ensure Hospice Care/patient/caregiver needs are met. Participates in special projects and performs other duties as assigned. Qualifications Licenses and Certifications: Hospice and Palliative Nurse (CHPN), Hospice and Palliative Credentialing Center preferred License and current registration to practice as a Registered Professional Nurse in the State of New York required Education: Associate's Degree in Nursing required Bachelor's Degree in Nursing preferred Work Experience: Minimum of two years of clinical nursing experience in Acute Care (Med Surg, ICU), Clinical Crisis Management, Hospice or Palliative Care required Clinical expertise and critical thinking skills required Effective oral, written, and interpersonal communication skills required Demonstrated expertise in clinical care management required Knowledge of personal computer operations including MS Office applications required Experience in health care management preferred Pay Range USD $93,400.00 - USD $116,800.00 /Yr.
