This may include a TytoCare-facilitated follow-up, an in-person NP visit, PCP/specialist follow-up, urgent evaluation, or emergency services based on clinical urgency. • The LPN does not independently perform medical diagnosis, independent triage, or independently establish the patient's plan of care. • Participates in special projects and performs other duties as assigned.
