About this Job: Join Our Hospital-Based Consultation-Liaison Psychiatry Team MedStar Union Memorial Hospital is seeking a full-time Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our hospital-based Consultation-Liaison Psychiatry Service. This is an excellent opportunity for a psychiatric provider who enjoys caring for medically complex patients and working collaboratively with medical and surgical teams. In this role, you will provide psychiatric evaluation, treatment, and consultation for patients throughout the hospital while partnering closely with physicians, nurses, and other members of the interdisciplinary care team. Position Highlights Full-time, Monday–Friday daytime schedule Hospital-based Consultation-Liaison Psychiatry position Provide psychiatric consultation for patients admitted to medical and surgical services Evaluate and manage psychiatric and behavioral health concerns in medically complex patients Collaborate closely with hospitalists, surgeons, nurses, social workers, therapists, and other members of the care team Diverse inpatient patient population with a variety of medical, surgical, and psychiatric presentations Support from an experienced multidisciplinary team Call schedule: one weeknight every two weeks and one weekend every eight weeks Weeknight call is telephonic Weekend call is primarily telephonic; in-person hospital evaluation is only required if a consult request necessitates it No outpatient clinic responsibilities What You’ll Do As a Psychiatric Mental Health Nurse Practitioner on the Consultation-Liaison Psychiatry Service, you will: Perform comprehensive psychiatric evaluations of hospitalized patients Assess and manage psychiatric symptoms and behavioral health concerns in the setting of acute medical and surgical illness Provide consultation regarding diagnosis, medication management, and treatment planning Evaluate and treat patients experiencing delirium, agitation, mood symptoms, psychosis, anxiety, and other psychiatric or behavioral concerns Develop individualized treatment recommendations in collaboration with the patient's medical team Initiate, adjust, and monitor psychiatric medications as clinically appropriate Provide recommendations regarding behavioral management and patient safety Communicate recommendations clearly with patients, families, and interdisciplinary care teams Collaborate with hospital physicians and clinical staff to support coordinated patient care Provide follow-up consultations as clinically indicated Document evaluations, recommendations, and treatment plans in the electronic medical record What We’re Looking For Master’s or doctoral degree in Nursing from an accredited program Board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP) Current Maryland APRN license or eligibility for Maryland licensure Prescriptive authority in Maryland or eligibility to obtain it Strong clinical assessment, communication, and interpersonal skills Interest or experience in hospital-based psychiatry, Consultation-Liaison Psychiatry, or medically complex patient populations Ability to work effectively within a multidisciplinary hospital environment Commitment to compassionate, evidence-based, patient-centered care Experience in Consultation-Liaison Psychiatry or inpatient psychiatry is preferred, but providers with strong inpatient psychiatric experience and an interest in Consultation-Liaison Psychiatry are encouraged to apply. #PL #LI-MP1 This position has a hiring range of : USD $115,000.00 - USD $190,000.00 /Yr. The Hospice Home Care Nurse reports to the Team Manager. He/she is responsible for identifying patient/family/facility care needs, and for providing nursing and supportive care for the terminally ill patient and their families. Serves nursing homes and assisted living facilities in Baltimore County (Owings Mills, Pikesville, Woodlawn....) Education: Diploma, ADN and/or BSN from an accredited institution Licensures/Certifications: • Current Maryland State license as a Registered Nurse Experience: Two years of nursing experience, or equivalent combination or education and experience. Skills: • Knowledgeable of universal precautions for infection control and practices safe nursing standards for self protection • Strong time management skills • Excellent interpersonal and communication skills • Ability to be patient and family focused • Skill in the use of EMR (Electronic Medical Record) and/or ability to learn Principal Duties and Responsibilities: • Completes initial assessment of patient and family to ascertain home care needs - including physical, emotional, spiritual and mental status. • Prepares and implements the care plan for patient/family unit in collaboration with the Interdisciplinary Group. • Provides direct nursing care to patients in order to maintain highest level of care for the patient in conjunction with the primary physician. • Plans and supervises the delivery of care by the HHA • Cooperates with Clinical Manager in utilization of other team members, outside agencies, and community resources. Updates interdisciplinary team at patient care conference • Uses problem oriented charting to ensure continuity of care in a home setting or nursing home as well as inpatient or respite setting. • Teaches family members, facility staff , hired caregivers, patient care skills, including: pain control, medication (RN titrates medication within prescribed range), injections, draws blood for labs, manages IV's, dressing changes, hygiene, nutrition, checking vital signs in accordance with orders from physician, as well as addressing emotional needs. • Ensures that arrangements for equipment necessary in the facility setting are made available. • Attends to patient, family and facility at time of patient’s death. Facility Specific Job Functions • Documents in facility chart • Maintains positive relationships and open communication with facility staff. • Reconciles medications against facility medication list. • Updates facility staff and family of visit schedule • Attends care plan meetings and/or patient care rounds for hospice patients within the facility. • Establish/maintain professional working relationships with a variety of facility staff; ED, DON, medicine techs, aides, nurses (LPNs and RNs), rehab staff, housekeeping, etc. • Provide educational in-services to facility staff both formally and informally. • Participate in marketing and customer service activities with BDMs. Pay Range $66,942.34 - $102,872.41 Final salary offer will be based on the candidate's qualifications, education, experience and alignment with our organizational needs.
