Perform assessment of potential patients consistent with Cleveland Clinic Hospice policies, procedures and admission criteria alongside the physician. Perform admission assessments for new admissions into contracted facilities and document information in the EMR within specified time frame. Plan the delivery of patient care in collaboration with other disciplines to provide comfort and symptom management. Perform daily assessment and coordinate the plan of care with inpatient staff for General Inpatients (GIP). Assess patient for safety risks, abuse and neglect. Assess and document pain using the appropriate pain scale and manage patient’s pain. Document changes in patient condition and effectiveness/timeliness of care/interventions. Evaluate, update and revise the plan of care to facilitate achievement of expected outcomes. Know action, usual dosage, indications and side effects of medications administered. Attend interdisciplinary meetings as assigned. While an interdisciplinary care team provides medical care to patients in long-term care facilities, you will have the opportunity to impact in a positive way the life and wellbeing of the elderly population. We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities. WOUND NP ROLE AND RESPONSIBILITIES: Provide wound care services for long-term care patients residing in skilled nursing facilities. Prescribe appropriate medications, including controlled substances in accordance with KY Law. Order, review, and/or follow-up with treatments and diagnostic testing. Communicate with the nursing home staff on a regular and as needed to discuss changes in condition. and/or the care plan. Collaborate with the primary care physician. WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS Current, unrestricted NP license in the state of Kentucky. Board Certification. DEA licensure (Optional) Clinical background in adult, family, or geriatric specialties. Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers. Must be comfortable staging pressure injuries according to the NPIAP Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc. Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices Strong preference to having wound certification or be open to getting certified Experience working in a long-term care setting. Ability to work with an EMR.
