SUMMARY: Under the management of the PACE SEMI’s Community Living Manager and the coordination of the Lead CLA, the Community Living Associate (CLA) provides supportive services to the participants (residents); including but not limited to the community setting, during transport and on appointments, or in the day health center while under the supervision of the manager, registered nurse, or lead CLA. The CLA performs a variety of tasks and services for the participants (residents), including: Personal care up to and including 2-person assist 1:1 interaction in DHC and CL as outlined in the plan of care Assistance with activities Assistance with meal preparation / cooking/ feeding/other nutritional needs Assistance with toileting / managing incontinence Simple treatments Grocery shopping Laundry services Coordination of care with participant and the DHC based on care plan Engage with resident elevating health concerns and customer services issues or needs appropriately Homemaking services / chores/ housekeeping services/sanitizing equipment All other duties as required by manager and lead CLA, as assigned Rounds q2 hour after 7pm-7am. Immediately reports all customer service complaints, all deviations from set schedule and appointments to supervisor /manager or lead CLA. The CLA responds to emergency situations in accordance with established policies and procedures. Works only within established scope of practice. KNOWLEDGE, SKILLS AND ABILITIES: H.S. diploma or GED required Valid Michigan Driver's license Current CPR card required Certification is optional though preferred, but must have one (1) year experience with a frail or elderly population Experience must be in a long-term care, hospital, home care setting, or PACE setting Required completion of a basic nursing assistant training program. As such, they are responsible for participant acute and chronic care management under direction and supervision of the center Physician-In-Charge (PIC). Job Summary This position is 4-month internship program which prepares the new Graduate nurse for a Registered Nurse position in 5D-SICU. Orientation includes a combination of classroom and precepted bedside clinical experience. A two (2) year commitment with a minimum of 36 hours/week (0.9 FTE) to the SICU is required after the completion of the 16-18 weeks of the orientation period. The orientation period will be a combination of day and nightshifts to meet orientation and learning needs. The nursing position will be a night shift position upon completion of the externship and orientation period. Responsibilities* 5D Surgical Intensive Care Unit: The SICU is a 20 bed adult intensive care unit caring for patients who have had major operations or those experiencing complications requiring extensive physiological monitoring. Typically this encompasses those patients with peripheral vascular diseases, cancer, hepatic, pancreatic or renal failure, cardiac and pulmonary disease. Often these patients present with multi-system failure requiring swan ganz monitoring, ventilator support, parenteral nutrition and fluid and electrolyte corrections. In addition, SICU receives adult liver transplants and is a major referral center for adult ECMO/Respiratory failure patients. This ICU also may receive overflow from the Trauma, Neuro, Medicine and Cardiac Intensive Care Units. SICU charge nurses participate as adult Rapid Response Providers through out the University Hospital. SICU offers 12 hour night shifts, comprehensive orientation, career development and the opportunity to work with a diverse patient population. Initiates and/or leads minimally one Daily Path engagement program provided for residents as described in the resident’s Life Story, Solace program or calendar. Encourages residents to socialize and participate in planned, spontaneous, individual or small group person centered programs and to support friendships between residents. Maintains residents' records daily, in a timely manner, and in accordance with company policy and procedures. This includes documentation related to medication distribution as applicable, engagement interests, incidents and observations, reporting any changes in resident's physical condition and/or behavior, visitors, etc. Observes and reports the health and emotional condition of each resident and promptly reports all changes to the supervisor. Only if permitted by state specific regulations, may assist residents with medication as defined in medication procedure. Follows physician orders and state laws to administer medications to residents who cannot self-administer their medications. Exhibits understanding of and follows medication policies and procedures.
