Takes and records vital signs as ordered and otherwise when necessary, i.e., monthly or daily as instructed by supervisor. Must be CNA Certified to do so. Notifies a supervisor at once if noting any adverse reaction of medication on resident or awareness of medication error. Observes and communicates to the Wellness Coordinator, Nurse/Executive Director any changes in resident physical or mental status. Assists with ADLs and assignment logs for residents whether they are on the assignment sheet or not and reporting any significant changes in residents or declines. Communicates, as needed, with physicians, families and other appropriate parties concerning resident status, changes in status, need for appointment, etc. Assist residents with personal hygiene and grooming twice a week, daily, or as needed. This includes: bathing, toileting, hair care, mouth care, skin care shaving, and nail care. Assist with ambulation on all shifts.
