SUMMARY: Under the management of the PACE SEMI’s Community Living Manager and the coordination of the Lead CLA, the Community Living Associate (CLA) provides supportive services to the participants (residents); including but not limited to the community setting, during transport and on appointments, or in the day health center while under the supervision of the manager, registered nurse, or lead CLA. The CLA’s responsibilities for each participant are specified in the CLA Care Plan. The CLA performs the care or services that are outlined in the individual participant’s care plan; any care guidelines that are pertinent to the participant’s diagnosis are specified in the care plan. The CLA performs a variety of tasks and services for the participants (residents), including: Personal care up to and including 2-person assist 1:1 interaction in DHC and CL as outlined in the plan of care Assistance with activities Assistance with meal preparation / cooking/ feeding/other nutritional needs Assistance with toileting / managing incontinence Simple treatments Grocery shopping Laundry services Coordination of care with participant and the DHC based on care plan Engage with resident elevating health concerns and customer services issues or needs appropriately Homemaking services / chores/ housekeeping services/sanitizing equipment All other duties as required by manager and lead CLA, as assigned Rounds q2 hour after 7pm-7am. The CLA is responsible for accurate timely documentation as required by the community living setting; including but not limited to the care and/or services provided during participant (resident) encounter(s). The CLA keeps the manager, lead CLA, and clinical team informed of the participant’s progress. Immediately reports all customer service complaints, all deviations from set schedule and appointments to supervisor /manager or lead CLA. The CLA responds to emergency situations in accordance with established policies and procedures. Works only within established scope of practice. KNOWLEDGE, SKILLS AND ABILITIES: H.S. diploma or GED required Valid Michigan Driver's license Current CPR card required Certification is optional though preferred, but must have one (1) year experience with a frail or elderly population Experience must be in a long-term care, hospital, home care setting, or PACE setting Required completion of a basic nursing assistant training program. SUMMARY: Under the management of the PACE SEMI’s Community Living Manager and the coordination of the Lead CLA, the Community Living Associate (CLA) provides supportive services to the participants (residents); including but not limited to the community setting, during transport and on appointments, or in the day health center while under the supervision of the manager, registered nurse, or lead CLA. Responsibilities* 6C Medicine Pulmonary Unit 6C is a 32-bed unit that provides 24-hour nursing care to adult patients. 6C is the home floor for Internal Medicine. Types of patients treated on 6C include pulmonary patients present with compromise of the cardiopulmonary system. Works collaboratively with leadership teams in the hiring process. Manages staffing and scheduling of nursing staff. Assists Senior Nursing Director in determining adequate complements of staff, scope of staff roles and appropriate workload. Manages or facilitates nursing relief staffing for multiple clinics. Provides supervision, counsel, discipline, and performance evaluation of nursing and as indicated, assigned clinical assistive staff, with feedback from medical faculty, peer groups, and ACU leadership teams. Manages the orientation and competency evaluation of nursing staff. Connects individually with each employee, increasing employee engagement scores and satisfaction. Clinical Practice Serves as a role model, mentor and expert resource to nursing and assistive staff in the provision of direct patient care. Provides leadership for the development, implementation and evaluation of standards of nursing practice and interdisciplinary clinical guidelines. Facilitates and supports a nursing practice environment in which empowerment, responsibility, autonomy, accountability, collaboration and peer review are exercised. Develops standard processes and protocols to minimize variation, support efficient workflows, and confirm the right work is performed by the right role. Support of Clinical Operations Participates in planning and problem-solving of daily unit operations and priorities of patient services. Participates in the development, implementation and evaluation of unit goals and objectives. Manages, directly and in collaboration with others, staffing, scheduling, benchmarking, work redesign and variances. Organizes and facilitates unit based educational activities and in-services. Assures compliance with regulatory programs' standards related to the nursing. Provides leadership, assists and advises Administrators, Medical Directors, in satisfying other regulatory standards. Interprets and implements Michigan Medicine policies and procedures. Assists in the planning and implementation of new or expanded services and programs. Provides interface and liaison communications regarding programs external to the unit which have implications for nursing staff. Represents nursing, Ambulatory Care Services and/or the clinical department(s) or unit(s) on assigned committees Quality Management and Research Participates in and conducts the analysis of data, assists in the development of strategies to improve care management within the area or unit. Participates in and supports the involvement of nursing staff in patient and ACU quality improvement metrics/activities. Provides leadership to staff in understanding and applying quality improvement principles and adoption of evidence-based changes in practice into daily work. Promotes the integration of research findings into clinical practice. Supports research activities on the unit, as indicated. Works closely with each team to define and measure productivity, holding teams accountable to expectations, and ensuring work performed is within their scope of practice. Professional Development and Education Facilitates the education of health sciences students. Develops self professionally through continuing education, professional reading, participation in professional organizations and networking. Evaluates own performance periodically with the Senior Nursing Director. Required Qualifications* Required Educational Requirements Applicants must meet one of the following: Bachelor of Science in Nursing OR Associate's degree or Diploma in Nursing and a Master's degree in Nursing. Current licensure as a Registered Nurse in the state of Michigan Minimum five (5) years RN experience Minimum two (2) years of recent (within the last 5 years) RN experience in pediatric service line. Leadership experience preferred Demonstrated leadership skills including creative and effective problem-solving, ability to effectively collaborate within a multidisciplinary team, and ability to direct nursing practice and apply critical thinking skills/judgment to a variety of situations. Nurse Leadership certification or completion of a nursing leadership certification within two (2) years of accepting this position. Certification is expected to be maintained while in this leadership role. Examples include Certified Nurse Manager and Leader (CNML) or, Nurse Executive (NE-BC). Evidence of demonstrated or potential leadership ability, including charge nurse, team leader or other leadership experience. Active participant in professional organizations Experience with Epic or MiChart Previous experience in the application of evidenced-based medicine in a clinical patient care environment is desired.
