Coach and mentor Charge Nurses, Staff Nurses, members of the Nursing Leadership Team and others. Clinical Practice Rotate to the off shifts as determined by staffing needs and the Clinical Nursing Director. Maintain significant presence rounding on patients, families and interfacing with staff to ensure patient care excellence on all shifts. To provide clinical assessment. Mentoring, coaching and evaluation of staff. Management of Clinical Area Participate in the development and implementation of unit or area goals and objectives. Identify and plan for staffing consistent with the needs of the patient population and budgetary requirements. Participate in planning and problem solving of daily unit/area operations, priorities of patient services, and urgent and emergent care. Interpret and implement the Hospital and University policies and procedures and the standards of regulatory agencies. Participate in the monitoring of Department payroll and commodity expenditures. Provide input into the payroll and commodity budget planning process. Participate in hiring activities of unit staff, conducting personnel interviews and reference checks. Coordinate performance evaluation activities for unit staff as delegated by the Clinical Nursing Director. Participate in developing of unit/area policy/procedure and standards of Nursing Care. Collaborate with Clinical Nurse Specialist and/or Educational Nurse Coordinator to implement organization, department and/or unit policies, procedures and standards of Nursing Care. Collaborate with Educational Nurse Coordinator to implement competency-based evaluation. Collaborate with intra- and inter- disciplinary, and intra- and inter departmental colleagues to create the Ideal Patient Experience. Anticipate and respond to patient/family issues and concerns. Quality Improvement and Research Participate in planning, coordinating, and evaluating unit quality improvement activities. Promote the integration of research findings into clinical practice of the unit's nursing staff. Promote staff involvement in clinical research. Required Qualifications* Required Educational Requirements Applicant must meet one of the following: Bachelors of Science in Nursing OR Associates degree or diploma in Nursing and a Masters degree in Nursing. Current licensure as a Registered Nurse in the State of Michigan. Minimum five (5) years RN experience. Minimum two (2) years of recent (within the last 5 years) RN experience in General Adult Medical-Surgical Nurse Leadership certification or completion of a nurse leadership certification within two (2) years of accepting this position. Certification is expected to be maintained while in this leadership role. Examples include Certified Nurse Manager and Leader (CNML) or, Nurse Executive (NE-BC). Job Summary The Adult Inpatient Psychiatry Unit (9C) offers inpatient diagnostic evaluations and comprehensive treatment plans to patients with serious psychiatric disorders. Treatment planning includes recommendations for appropriate psychotherapeutic, psychosocial and psychopharmacological (medication) treatments, with vocational treatments following discharge. We emphasize coordinating these recommendations with ongoing or subsequent psychiatric management. Patients also may be invited to participate in a variety of approved diagnostic and treatment research programs. We are actively engaged in research in the areas of schizophrenia, psycho-immunology and affective disorders. 9C offers a creative staffing model to their nurses to promote optimal continuity of care for their patients. SUMMARY: The Clinic Registered Nurse Coordinator serves in the role of Clinical Coordinator responsible for ensuring the functions optimally by appropriately delegating and monitoring the activities of the Licensed Practical Nurse, Medical Assistant and Scheduling Clerks. The Clinic RN Coordinator demonstrates a direct relationship between nursing interventions and participant outcomes, demonstrates clinical competence and engages in effective patient teaching for acute and chronic illnesses. The focus of care is one that enhances functional capacity, encourages autonomy in all aspects of care, and encourages Participant, Caregiver and Family involvement in achieving health care goals. SPECIFIC DUTIES AND FUNCTIONS: Collaboration with the Physician and Nurse Practitioner to ensure optimal clinical flow. Daily workflow of clinical activities and delegation of task to the clinic Licensed Practical Nurse, Medical Assistant and Scheduling Clerk. Triaging and assessment of Participants in the clinic setting and providing education resources. Supervises the LPN and Medical Assistants to ensure competency of clinical skills and task. Collaborates with Manager to ensure adherence to quality initiatives in the clinic setting and organizational. Ensures that clinical activities are documented in the Participants medical record by clinic staff daily. Ensures that documentation of clinical activities are evidence-based and adheres to standards set by quality and professional standards. Collaborates with RN Case Managers regarding care delivered to Participants in the clinic and any transitions of care, through verbal and electronic communication. Collaborates with Nurse Educators regarding any assessed educational needs of staff and new hire competency skills compliance. KNOWLEDGE, SKILLS AND ABILITIES: BSN preferred. At least one year working with the frail elderly population. Five years of management experience preferred. Must have the ability to lead and direct staff in the delivery of care. Must be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact. Excellent oral and written communication skills necessary; must have strong interpersonal skills. Must have the ability to understand and standardize process related to quality indicators and utilize technology to document and present data. SUMMARY: Under the management of the PACE SEMI’s Community Living Manager and the coordination of the Lead CLA, the Community Living Associate (CLA) provides supportive services to the participants (residents); including but not limited to the community setting, during transport and on appointments, or in the day health center while under the supervision of the manager, registered nurse, or lead CLA. The CLA’s responsibilities for each participant are specified in the CLA Care Plan. The following statements are intended to describe the essential elements, functions, and requirements of the position. The list should not be taken as an exhaustive list of all responsibilities, duties, and skills required of an individual assigned to this job. SPECIFIC DUTIES AND FUNCTIONS: The CLA plan of care is based on the initial and on-going assessments that are performed by the manager, assigned RNCM/clinical team, and lead CLA. The CLA assists in providing and/or maintaining optimal physical and emotional comforts to the participants (residents) living in the community setting while embracing the culture, mission, values, and the practicing Ten Principles at PACE SEMI. The CLA assists in creating a nurturing supportive environment while helping participants (residents) achieve maximum self-reliance and independence within the community living setting. The CLA performs the care or services that are outlined in the individual participant’s care plan; any care guidelines that are pertinent to the participant’s diagnosis are specified in the care plan. The CLA performs a variety of tasks and services for the participants (residents), including: Personal care up to and including 2-person assist 1:1 interaction in DHC and CL as outlined in the plan of care Assistance with activities Assistance with meal preparation / cooking/ feeding/other nutritional needs Assistance with toileting / managing incontinence Simple treatments Grocery shopping Laundry services Coordination of care with participant and the DHC based on care plan Engage with resident elevating health concerns and customer services issues or needs appropriately Homemaking services / chores/ housekeeping services/sanitizing equipment All other duties as required by manager and lead CLA, as assigned Rounds q2 hour after 7pm-7am. The CLA is responsible for accurate timely documentation as required by the community living setting; including but not limited to the care and/or services provided during participant (resident) encounter(s). The CLA keeps the manager, lead CLA, and clinical team informed of the participant’s progress. Immediately reports all customer service complaints, all deviations from set schedule and appointments to supervisor /manager or lead CLA. The CLA responds to emergency situations in accordance with established policies and procedures. Works only within established scope of practice. KNOWLEDGE, SKILLS AND ABILITIES: H.S. diploma or GED required Valid Michigan Driver's license Current CPR card required Certification is optional though preferred, but must have one (1) year experience with a frail or elderly population Experience must be in a long-term care, hospital, home care setting, or PACE setting Required completion of a basic nursing assistant training program.
