Overview The official job title for this position at PPGT is “Clinician”. The Clinician provides comprehensive reproductive healthcare services to patients at Planned Parenthood of Greater Texas (PPGT). Works as part of the Health Services team in collaboration with Health Services and other team members to meet the clinical needs of patients as outlined in affiliate protocols. Orders, interprets, and records results of clinical tests and consults with supervising physicians as needed. Prescribes medications and makes recommendations for other forms of treatment, including contraception. The Clinician is part of the medical services team providing sexual and reproductive healthcare as outlined in affiliate protocols. Provides services by telehealth and/or in-person visits including: history review, health education, physical exam, counseling, diagnosis, and treatment according to protocols. The Clinician works in partnership with management, Health Services and Chief Operating and Medical Services Officer (COMSO) to provide the highest quality of patient care and excellent patient experience. Responsibilities • Performs physical examinations with special emphasis on the reproductive system including breast examinations, pelvic/genital, cancer screening tests, diagnosis of sexually transmitted infections (STIs), and other types of more specialized services or procedures as may be indicated by medical policy or protocols (e.g. gender affirming hormone therapy-GAHT). • Orders, and interprets diagnostic studies as indicated and permitted by medical protocols. Performs lab testing per Clinical Laboratory Improvement Amendments (CLIA) regulations and according to PPGT’s lab manual or manufacturer’s directions. • Provides patient care according to all Medical Standards and Guidelines and/or the specific direction of a supervising physician. Consults with organization COMSO or designee when deviates from PPGT Medical Standards and Guidelines (MS&Gs). • Provides relevant health instruction and education to include family planning, sexual counseling, and principles of health promotion • and maintenance. • Documents exam findings and other clinical aspects of direct patient care into the medical record accurately. • Codes billable services accurately based on medical record documentation using accepted practices, i.e. Evaluation and Management (E&M), Current Procedural Terminology (CPT) and diagnosis codes (ICD-10). • Ensures conformity with the policies and procedures of the affiliate, Texas Medical Practice Act, Physician Assistant Licensing Act, Texas Nurse Practice Act, and rules promulgated under those acts. • Recognizes ethical, legal, and professional issues inherent in providing care to health center patients throughout the life cycle. • Initiates and monitors appropriate follow-up according to PPGT Medical Standards and Guidelines on all abnormal test results and referrals. • Remains well informed about current contraceptive technology and reproductive and sexual healthcare issues. • Develops and maintains a collaborative working relationship with the Health Center Manager and support staff resulting in a team effort in meeting center goals. • Develops a level of time management that is conducive to seeing 3-4 clinician visits per hour meeting productivity goals and expectations. • Takes an active role in managing patient satisfaction by practicing patient centered care and providing exemplary customer service to patients. • Responds to medical emergencies as provided in the medical protocols and is responsible for maintenance of emergency box • May assist in supervising and participating in the orientation and proctoring of new clinicians. • Administers, supplies, or prescribes medications/devices, including injections, per delegated orders of the Pharmacist-in-charge and the Chief Medical Officer as outlined in The Manual of Medical Standards and Guidelines and Pharmacy Manual and as per Title 22, Part 11, Chapter 22 of the Texas Administrative Code. • Maintains an active Prescriptive Authority Agreement which is reviewed with authorizing physician annually. Summary The Department of Internal Medicine, Section of Hematology/Oncology is seeking a qualified candidate for an Advanced Practice Provider (APP) position in an inpatient hematology role at the Dan L Duncan Comprehensive Cancer Center at Baylor College of Medicine in the Texas Medical Center. This position is open to either a Nurse Practitioner (NP) or Physician Assistant (PA). The APP will provide medical care to patients with hematologic disorders in the inpatient setting under the direction of a supervising physician. The candidate will be expected to demonstrate clinical excellence, scholarly enthusiasm, and a track record of high-quality clinical performance. Job Purpose The Inpatient Hematology APP will provide advanced clinical care for hospitalized patients with hematologic disorders in collaboration with attending hematologists, hematology-oncology fellows, and the multidisciplinary inpatient care team. Core responsibilities include managing admissions, performing daily clinical assessments, developing and implementing treatment plans, coordinating inpatient care, and discharge planning. The APP will work closely with nursing staff, outpatient hematology APPs, care navigators, and ancillary services to ensure safe, evidence-based, and patient-centered care throughout hospitalization, including coordination of complex discharges. The role includes participation in daily inpatient rounds, weekly intake meetings, and monthly hematology tumor boards. As appropriate to training, credentialing, and institutional scope of practice, the APP may also have the opportunity to perform hematology-related procedures, including bone marrow biopsies and lumbar punctures with intrathecal treatment administration. Job Duties Obtain detailed patient histories and perform physical examinations. Perform, order, and interpret diagnostic procedures in accordance with patient health concerns. Summary The David J. Sugarbaker Division of Thoracic Surgery in the Michael E. DeBakey Department of Surgery is seeking a qualified Advanced Practice Provider to provide high quality surgical care under the direction of a group of physicians at Baylor St. Luke’s Medical Center. Candidate will be expected to provide clinical excellence and scholarly enthusiasm. The provider will have direct patient care responsibilities. The selected candidate will be appointed as a faculty Instructor and be eligible for full faculty benefits. Job Duties Primary responsibilities include inpatient care of thoracic surgery patients in coordination with attending physician and resident physician teams. This will include thorough pre-round chart review, engagement in collaborative rounds with attending physicians as well as independent rounding, documentation of clinical decision-making and treatment plans, coordination and execution of the day’s care plan, including wound management and other procedural interventions as needed, and frequent follow up on pt progress and testing to ensure patients are progressing through their plan of care. Obtain detailed history and perform physical examinations. See consults independently and/or with residents/fellows in various settings – floor, ICU, ED, etc. Formulate assessments and plans and evaluate the patient response to care Order and interpret appropriate diagnostic procedures, including, but not limited to x-rays, electrocardiograms, laboratory tests, and MR and CT imaging Ensure timely and accurate documentation in the EPIC electronic medical record system Prescribe appropriate pharmacological and non-pharmacological treatments (while inpatient and upon discharge) Educate patients on prescribed therapeutic regimens, home care and health maintenance Remove chest tubes and JP drains as indicated, provide wound care to chest tube sites and surgical wounds Perform bedside simple wound debridement, suture closure, and bedside intrapleural therapeutic procedures Provide wound care and education on cervical esophagostomies, negative pressure wound therapies (wound vac), feeding tubes, indwelling pleural catheters, and tracheostomies to patients and staff Provide guidance and work direction to support staff, including, but not limited to nurses, social workers, and clinic coordinators Optimize discharge planning for all patients including participation in daily multidisciplinary rounds for floor and ICU patients, as well as timely and clear discharge communication to outpatient team Communicate with physicians, patients, and the family members of patients concerning patient care Provide orientation, direction and support to PA/NP students, medical students, and/or residents assigned to the service Promote a positive work environment with a team-based approach through supportive and respectful interactions with peers, staff, learners, and faculty Other duties as assigned Minimum Qualifications Education Required: Master’s degree in in Nursing. Experience Required: None required. Certification/Licenses/Registration: Current license as a Registered Nurse (RN) by the State of Texas Board of Nursing. Current Texas Licensure as an Advanced Practice Nurse (APRN-NP), and specialty certification as required by department.
