Assignment will be by units/clinical areas of practice and may require responsibility on other units/services Responsibilities 1. Principle Duties and responsibilities include, but are not limited to: a. Reviews all new admissions to identify patients where utilization review, discharge planning, and/or case management will be needed using standardized criteria to achieve optimal patient outcomes and appropriate reimbursement for the organization. b. Performs continued stay reviews utilizing standardized criteria to justify continued inpatient stay. c. Collaborates with Physician and other clinicians to expedite diagnostic testing, treatment and consultations. d. Documents all clinical reviews in Case Management Documentation system. e. Supports the mission, vision, philosophy and goals of the Medical Center. 2. Case Management: The CM process will include: a. Assessment of the patients clinical, psychosocial, and functional status in collaboration with the interdisciplinary team. b. Identification and documentation of variances affecting the LOS and the discharge planning process. c. Conducts follow-up of any delays in treatment or reporting of results. d. Planning/developing specific goals with the physician, interdisciplinary team, and the patient and/or family. e. Documentation of outcomes achieved and identified internal and external barriers. g. Identifies reasons for readmissions and collaborates with interdisciplinary team on strategies to reduce readmission rate. h. Interacts with patient/family to discuss plan of care and coordination of services based on clinical needs and available resources. 3. Utilization Review: a. Maintains a working knowledge of the UR (Utilization Review) requirements of each payor within the patient population; b. Provides the clinical information requested by the managed care companies as part of the concurrent review in a timely fashion. c. Provides clinical information requested by the managed care companies in accordance with contractual agreements. d. Works collaboratively with physicians and managed care companies on concurrent denial appeals e. Communicates clinical information to the payor, as needed, coordinating direct communication between physician and payor Medical Director as required. f. In psychiatry, the CM will document UR notes in the medical record as per OMH guidelines. 4. Discharge Planning: a. Responsible for assessment, communication and monitoring of discharge planning process (The clinical nurse initiates the discharge planning process on admission). b. Obtains authorizations from managed care companies for post-discharge services. c. Assesses for clinical readiness and completes the Hospital and Community Patient Review Instrument (PRI) for patients requiring Residential Health Care Facility placement. d. Liaison with financial department for current insurance coverage. 5. Collaborates and participates in the appeals process with all members of the interdisciplinary team. 6. Collaborative Relationships: a. Develops and maintains effective working relationships with interdisciplinary team and Case Managers within the managed care organizations. 7. Responsibilities Assesses all patients assigned and establishes and implements the Plan of Care Adheres to policies and procedures addressing the scope of services outlined in both Infusion nursing services, pharmacy services and state laws and regulations Adheres to INS and IgNS standards of practice, CDC guidelines, scope and standards of practice, and code of ethics as defined by American Nurses Association Collaborates with pharmacy staff, sales partners, ordering provider and patient to develop and monitor goals of patients’ therapies Completes patient care documentation in place of service including patients’ home or AIS Manages vascular access devices, to include insertion of peripheral infusion devices and ability to draw labs via peripheral venipuncture Educates patients and caregivers Evaluates progress toward goals and makes recommendations to physicians for changes in the Plan of Care based on patients’ needs Coordinates nursing care with home health and other community agencies as indicated Participate in on-call rotation as scheduled per branch protocol Complete expense reports monthly tracking mileage and other miscellaneous incidental expenses Complete annual competencies and mandatory training within assigned due dates Supervisory Responsibility: No Qualifications Graduate of an accredited school of nursing; Bachelor of Science in Nursing preferred Valid nursing license in state(s) of practice Minimum of one (1) year of experience as a Registered Nurse Previous infusion and home health care experience preferred Extensive experience in IV therapy preferred Current CPR certification Flexible schedule with the availability to work a variety of shift models Ability to work effectively within a multidisciplinary team Effective verbal and written communication skills Valid driver’s license and auto insurance Must meet company standards for driving record as well as driver age requirement Percentage of Travel: 100%-Regular driving to patient’s homes for visits is required as part of this position **To perform this role will require constantly sitting, standing, walking, bending, reaching, crouching, stooping and typing on a keyboard with fingers. ESSENTIAL JOB DUTIES AND RESPONSIBILITIES SRH Clinical Care and ICA Patient Care (90%) · Reviews the patient health history for completeness and records findings. Alerts APC regarding critical information about the patient’s chief complaint, history of present illness, and other current or past medical or social problems. · Provides care to patients receiving family planning, abortion, colposcopy, LEEP, and other GYN services according to PPGNY medical policies and procedures under the direction of APC, or physician. · Provide direct nursing care throughout the abortion care continuum—pre-procedure, intra-procedure, and post-procedure—including preparation, continuous monitoring, and recovery. Assist in the procedure room and recovery area, ensuring clinical stability and emotional support for patients. Administer and monitor medications used to induce sedation in accordance with PPGNY protocols and RN scope of practice. Ensure safe, timely documentation, patient education, and post-sedation evaluation. · Accurately documents and administers medications as ordered by the provider and provides associated teaching. · Documents competently in the electronic medical record using the correct PPGNY templates and forms. · Arranges for interpreters for patients with communication barriers. · Collaborates with members of a multi-disciplinary health care team & provides care within New York State RN scope of practice. · Assists in promoting optimum center operations, excellent patient service and compliance with both agency and funding source policies and regulations. · Participates in health center/affiliate efforts to achieve established goals for productivity. · Adheres to affiliate goals and policies on professionalism, wait time in-clinic and on the phone, and the system for addressing client complaints. · Assists in the orientation and training of new RN/LPN or unlicensed medical personnel on the center team as needed. · Performs other educational and experience related duties as required. · Assists in abortion service in a variety of functions including abortion counseling, · Provides health education and nursing care related to family planning, reproductive health, abortion service, transgender health services, and STI’s to all clients. · Provide accurate patient centered, non-judgmental, education according to MS&G and signs informed consent related to visit. · Performs laboratory tests as ordered, completes laboratory requisitions, packages labs for transport according to PPGNY lab procedures. Performs daily controls per lab protocols. · Follows all safety and security procedures and guidelines. · Participates in agency infection prevention and quality control initiatives and protocols. · Notifies patients of laboratory results and educates regarding clinician plan for follow up. · Screens and follows up on patient phone calls in collaboration with center APC or PPGNY physicians. Maintenance of Clinical Skills and Competencies (10%) · Maintain licensure, certifications, and competencies in abortion care and reproductive health as required by PPGNY, PPFA, NYS DOH. Participates in regular competency checks and training for all new services, tests, and skills as assigned. Performs and maintains job duties within scope of practice based on RN competencies, NYS Licensure, BLS, ACLS, PPFA Privileging and/or specific education and training.
