Description A nursing student, who has completed the first clinical year, inclusive of medical – surgical nursing, within a baccalaureate nursing program and has a fundamental knowledge of the art and science of nursing, can function in this role. This experience assists the student nurse in expanding previously learned theory and skills through supervised patient care within a work setting. Responsibilities Roles & Responsibilities: A nursing student, who has completed the first clinical year, inclusive of medical – surgical nursing, within a baccalaureate nursing program and has a fundamental knowledge of the art and science of nursing, can function in this role. This experience assists the student nurse in expanding previously learned theory and skills through supervised patient care within a work setting. Performs and records patient’s vital signs, such as blood pressure, pulse, temperature, respirations, pulse oximetry and pain rating. Documents clinical data as assigned, in a computer or manual system. Performs clinical procedures, such as specimen collection, venipuncture, EKG, application of sequential compression sleeves, post-mortem care, maintenance of skin integrity and capillary blood glucose monitoring. Sets up for selected procedures and assists RN's accordingly. Assists patient with activities of daily living, such as feeding, bathing, and/or toileting. Orients patients to Specialty Service/Unit environment. Assists patient with ambulation and all levels of mobilization. Assists with dietary functions, including delivering and collecting patient trays at bedside, passing menus to patients and assisting them with menu completion, mixing nutritional supplements, reporting variation in eating patterns to RN/Dietitian. Maintains accurate and timely systems for transmission of patient dietary orders and changes. Assists in pre- and post-operative procedures, such as patient transport, completion of pre-op checklist and reception of patient from PACU/OR. Utilizes measures to insure patient safety and promotes a restraint free environment. Maintains use of restraints for patients under the direction of a RN and according to set policies and procedures. Provides care to the deceased as well as the involved family/significant other. Assists the professional registered nurse in the collaborative plan for patient education participating in patient/teaching activities. Maintains a safe, clean and functional environment, including ensuring for the comfortable movement of the patient. Assists the patient in room maintenance, such as bed-stripping and trash removed, when necessary. Performs one-to-one observation of patients on constant observation status, e.g., for safety, elopement, or suicide prevention. Performs correct infection control techniques and maintains isolation status for patients under direction of a RN and according to set procedures. Performs One-to-One/Constant Observation and Safety Observations of patients as ordered, e.g., for safety, elopement, or suicide prevention. Transports patients to stretchers or wheelchairs, as required. Operates the pneumatic tube system when required. Performs other duties as assigned. Qualifications Requirements: Education: High School Diploma / Associate Degree. BLS from American Heart Association required. Completion of the first clinical year in an accredited baccalaureate program in Nursing, with experience in a medical surgical rotation. Grade point average of 3.5 or above. Satisfactory nursing faculty recommendations. This position takes on some of the responsibilities of the nursing department as may be delegated. He/she provides assistance in the functioning of the nursing department. In addition, he/she assists and supports the translation of the nursing philosophy of the facility into nursing practice by participating in the planning, implementation, and evaluation of the nursing care delivery system. RESPONSIBILITIES/ACCOUNTABILITIES: 1 Assists and participates in the development, organization and implementation of the philosophy, objectives, policies, procedures and standards of nursing care in accordance with the goals of Windsor Gardens Care Center; 2 Confers with the Director of Nursing in assessing the quality of nursing care being delivered and recognizes the need for improving or changing nursing practices; 3 Assists with the development, direction, participation, and evaluation of the orientation program for nursing personnel; responsible for orientation of new nursing employees in the absence of the Director of Staff Development; 4 Makes daily rounds to evaluate resident care, the progress of individual employees, monitors nursing practices and assists personnel with nursing and educational needs; 5 Remains cognizant of legal aspects of nursing practice as well as government regulations; demonstrates this in teaching, supervision, and evaluation of resident care; 6 Assists with on-site evaluation of employee performance and assesses need for progressive disciplinary action; participates in employee conferences, as requested by the Director of Nursing; 7 Concerns his/herself with the safety of all facility residents in order to minimize the potential for fire and accidents. Also, ensures that the facility adheres to the legal, safety, health, fire and sanitation codes by being familiar with his/her role in carrying out the facility’s fire, safety and disaster plans and by being familiar with current MSDS; 8 Assists with daily scheduling to ensure adequate and safe staffing necessary to deliver nursing services; keeps within predetermined par level staffing ratios; 9 Acts as on-call nursing clinical resource person, as scheduled; 10 Participates in internal audit of nursing practice and in the plan of correction when deficits are identified; 11 Compiles end-of-month reports to be submitted to the DON; 12 Establishes and facilitates effective employer/employee relations; 13 Assures that MDS, RAPS and care plans for the residents on the unit are completed in a timely manner and comply with all nursing facility/regulatory policies; 14 Acts as a liaison for residents, families, visitors and staff to resolve concerns and issues, always with the goal of resident satisfaction; 15 Participates in the Q.I. program of the nursing facility; 16 Ensures that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals’ needs and rights; 17 Performs other duties as requested. SPECIFIC EDUCATIONAL/VOCATIONAL REQUIREMENTS: 1 A graduate of an accredited School of Nursing with a current registered nurse licensure by the New Jersey State Board of Nursing. 2 A minimum of three years full-time or equivalent clinical experience is required and a minimum of two years of clinical experience in long-term care nursing with one year in a management/administrative or supervisory capacity is preferred. 3 BSN preferred. If a resident requires medication prior to breakfast, PCA shall go to the resident's room and assist the resident with medication. If a resident requires afternoon or after dinner medication, the PCA shall assist the resident with medication from the med room. PCA must always watch residents ingest medication and properly document. PCA shall update medication administration record as needed, after Physician visits & ensure MAR & doctor's orders match PCA shall ensure that resident receives all necessary documents prior to doctor appointments. Consult and correspond with doctor offices for any necessary clarifications.. Record all pertinent information re: residents in the 24 hr. Log book. Personal Care Assistance: PCA shall assist residents in the dining room during mealtime by distributing meals to residents. PCA shall assist with cleaning the dining room after meals when required. PCA shall distribute facility provided snacks to residents during evening snack time. PCA shall assist residents with showering/bathing as needed. PCA shall assist residents with maintaining a healthy hygiene routine as needed. PCA shall assist residents by laundering residents clothing, linens, and towels as needed. PCA shall assist residents that require limited mobility assistance as needed. PCA shall answer all call bells, and check residents' whereabouts during rounds. PCA shall assist residents with operating the elevator lift. PCA shall assist residents by performing housekeeping and light maintenance tasks as needed. These tasks include but are not limited to: laundering residents clothing, linens, and towels, cleaning resident rooms, making residents’ beds, unclogging toilets, adjusting room temperature, salt areas outside of facility.
