Description The Clinical Program Manager is responsible for assuring compliance with mandated clinical orientation, competency assessment and interventions in the assigned clinical program of designated nursing staff within the Medical Center. This individual interfaces directly with MSMC Departments, Administrators, Nursing, Medical and Quality Leadership, and Clinical Staff, managing the educational and operational impact of all clinical care delivery, staff performance, and outcome measures to support clinical practice. Overview Provides high quality comprehensive nursing care to meet the needs of the patient/client and manages/coordinates the delivery of cost effective multidisciplinary health care services for a caseload of patients/clients consistent with VNS Health philosophy, policy, goals and objectives, and Standards of Nursing Practice. Works under moderate supervision. For Nurse-Family Partnership (NFP) only: Provides health education, social support and community linkages to meet the needs of pregnant and postpartum, first-time mothers and their infants. Also responsible for adhering to NFP national guidelines. • Assesses the physical, social and psychological needs and environmental conditions of the patient/client through professional knowledge, skills of observation, interviewing, and physical examination to develop a plan of care. • Understands, interprets, and implements reimbursement protocols; determines the billing source for patient/client care. Consults with management on complex/unique reimbursement cases. • Acts as the coordinator of care for the multidisciplinary team by providing and managing the care needs of patients/clients which includes making/confirming referrals to other disciplines/community agencies and ordering supplies/services and ensuring they are delivered as needed. • Documents content and process of all services provided in accordance with government regulations/requirements, and VNS Health policies, practices, and procedures, as well as NFP requirements as appropriate. Initiates and maintains written (including clinical and progress notes) and verbal communications with team members and management to assure optimal patient/client care. • Participates in the orientation of selected staff through ‘buddy system’ field observation of staff and feedback to appropriate staff involved in orientation process. • Participates in selected student programs by providing guidance, training and mentoring to students. • Provides nursing care in accordance with VNS Health policies, practices, procedures and Standards of Nursing Practice, which may require standing, stooping, sitting, crouching, bending and stretching to deliver patient/client care. • Transports and utilizes required medical equipment and other supplies using VNS Health designated/supplied carrying case weighing approximately 25 – 30 lbs. to and from patient/client homes/care facilities. • Travels to patient/clients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient/client. • Follows work procedures in relation to established workflows. • Maintains productivity sufficient to meet VNS Health goals. • Assumes responsibility for personal professional growth through attendance at in-services, training, continued formal education and select journal readership. • Performs other related duties and participates in special project, as assigned. • For CHHA and Hospice Only • Establishes, implements, and revises a plan of care with input from appropriate multidisciplinary health team members (i.e., physician, OT, PT, SW, SLP, family members, caregivers, HHAs, etc.) based on goals mutually acceptable to the patient and significant others. Ensures that services follow the plan of care by ongoing assessment, planning, coordination, counseling, teaching and supervision of the delivery of care to meet the needs of the patient. • Oversees/supervises the care provided to the patient by Licensed Practical Nurses and Paraprofessionals through regular home visits in compliance with regulatory and VNS Health requirements. Assesses the patient’s condition, monitors the effect of the care plans on the patient/family health status, informs physician and other team members of changes in the client's needs and updates the plan of treatment, as needed. • Manages, plans, coordinates and executes the patient’s safe discharge from service. Ensures that physician’s orders, discharge summary and communication to team members, patient and family members adhere to VNS Health and regulatory requirements. • Communicates and consults with patient’s MD and follows-up on problematic situations affecting care such as abnormal clinical findings, medication discrepancies, or other factors that impact patient’s ability to comply with the plan of care. Manages and coordinates all aspects of assigned caseload. • Assists patients with activities of daily living, which may require positioning, moving, transferring and lifting patients of varying weights and physical conditions, with and without assistance and as appropriate, from family members and/or paraprofessionals. • For NFP Only • Establishes, implements, and revises a plan of care with input from the client and from appropriate multidisciplinary health team members (i.e., social worker, physician) based on goals mutually acceptable to the client. Ensures that services follow the plan of care by ongoing assessment, planning, coordination, counseling, teaching, and evaluation of the delivery of care to meet the needs of the client. • Manages, plans, coordinates and executes the client’s safe discharge from service. Ensures that discharge summary and communication to team members, patient and family members adhere to VNS Health and regulatory requirements. • Maintains a flexible work schedule to meet clients needs. • Participates in all required NFP training sessions. • Participates in special projects and performs other duties as assigned. Description Job Title: Patient Care Associate-Rehab-Full Time Day 7am-3:15pm-MSM As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Responsibilities A. Clinical/Technical/Service • Demonstrates the ability to perform clinical/technical/service/administrative tasks • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. • Performs and records patient’s vital signs, such as blood pressure, pulse, temperature and respirations. Documents intake and output and other data as assigned • Obtains patient’s rating on pain scales as a fifth vital sign in all areas except Pediatrics • Performs clinical procedures, such as specimen collection including blood glucose, venipuncture, EKG, application of compression devices, post-mortem care, and maintenance of skin integrity. • Sets up for selected procedures and assists RN’s accordingly. • Assists patient with activities of daily living, such as feeding, bathing and/or toileting. Orients Patients to clinical environment. Assists patient with ambulation and all levels of mobilization. • Assists with dietary functions, including delivering and collecting patient trays at bedside, passing menus to patients and assisting them with menu completion, mixing nutritional supplements, reporting variation in eating patterns to RN/Dietitian, and maintaining accurate and timely systems for transmission of patient dietary orders and changes • Maintains a safe, clean and functional environment, including ensuring for the comfortable Movement of the patient. Assists the patient in room maintenance, such as bed-stripping and trash removed, when necessary • Performs on-to-one observation or close observation status of patients, e.g. safety, elopement risk, or suicide prevention. • Maintains “isolation status” for patients under direction of an RN and according to set procedures • Performs scheduled hourly care checks/ rounding on patients with emphasis on caring for toileting needs, comfort, and positioning and pain awareness. Notifies RN if rounding has not been completed. • Utilizes Vocera system/Mount Sinai phone according to sign in/sign out protocol where activated. • Performs other duties as assigned, including generating reports • Is open and responsive to the diverse backgrounds and experience of other people and promotes an environment that is sensitive to cultural diversity. • Respects and values each patient’s unique, individual needs, beliefs, and diversity and incorporates these into delivery of care. • Greets visitors and answers telephones promptly and courteously. Escorts visitors to appropriate area, when necessary. Provides information as appropriate. • Answers Patient Call System and refers calls to appropriate staff. • Maintains a safe, clean and functional environment, including removing trash and bringing physical plant and equipment problems to the attention of the managers. • Transports patients to stretchers or wheelchairs, as required • Delivers and collects trays and bedside, delivers flowers to patients. • Strips units/cubicles • Performs emergency clean-up • Performs emergency transport • Performs discharge bed making • Transport specimens. • Updates communication board/discharge log • Assembles medical records • Updates Admission/Transfer/Discharge System/Processes • Maintains supplies and care of equipment. B. Organizational/Managerial • Participates in the department’s performance improvement activities. • Maintains patient/employee confidentiality in the management of information. • Observes the Health Care System’s compliance policies. C. Educational/Professional Development • Participates in the development of other staff members. • Meets regulatory, licensure and annual health assessment requirements. • Identifies learning strengths and needs • Utilizes learning resources. D. Communication/Relationships • Demonstrates a professional, courteous, and respectful attitude in dealing with patients, families and significant others. • Displays courtesy, tact and patience during interactions with all members of the hospital staff and extended community. Qualifications • High School Diploma (graduate or equivalent). • Certificate of completion of a medical assistant/patient care assistant program/equivalent with skills in venipuncture and EKG. • Interpersonal skills to communicate and collaborate effectively with other members of the Specialty Service. • Documentation of Medical Assistant/Patient Care Associate training • Three to nine months of healthcare experience preferred. • Ability to set priorities, problem solve, use proper judgement in difficult situations and be flexible • Sufficient oral and writing skills in the English language in order to communicate effectively. • Physical stamina and manual dexterity to perform a variety of tasks and skills in the area of assignment • American Heart Association Heartsaver or BLS Certification Employer Description Strength through Unity and Inclusion The Mount Sinai Health System is committed to fostering an environment where everyone can contribute to excellence. Compensation The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $27.8502 - $29.8773 Hourly. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
