Documents patient information accurately and in a timely manner in the EHR. Administers vaccines, injectable medications (including long-acting injectable medications for HIV), inhaled respiratory therapies (e.g. nebulized therapies) and provides wound assessment and wound care as indicated; provides medication adherence assessments and counseling, and when directed, delivers directly observed therapy. Provides chronic disease management support for HIV as well as other common co-morbid medical conditions such as diabetes and hypertension. Identifies high risk patients and works with the appropriate services and interdisciplinary team members to achieve the best possible patient care and outcomes. 7. Promotes shared decision making with patients in regards to healthcare; educates the patient and /or family regarding the patient’s clinical condition, treatment, and their role in their healthcare decisions. 8. Utilizes a variety of telecommunications technologies to assess, triage, provide nursing consultation, and perform follow-up and surveillance of patient status and outcomes . 9.
